Abstract:

Pro konkrétní organizaci poskytující zdravotnickou péči je stěžejní způsob, jakým je organizován celý systém zdravotnictví. Jednotlivé státy přistupují k zdravotnictví rozdílným způsobem. Rozdíly nalezneme zejména v oblasti financování. Spojené státy americké mají systém zdravotní péče založený převážně na soukromých zdrojích financování. Oproti tomu většina evropských států financuje zdravotnictví z veřejných zdrojů. V České republice je systém financování zdravotnictví založen na veřejném zdravotním pojištění. Začíná být ale stále více zřejmé, že neustále se rozvíjející se medicína bude potřebovat stále více finančních prostředků. Jedním z možných řešení je zavedení spoluúčasti pacientů. Ta by se realizovala formou přímých plateb nebo soukromého zdravotního pojištění, případně kombinací obojího. Nejenže by tak v systému zdravotnictví přibylo finančních prostředků, ale rovněž se dá očekávat snížení spotřeby zdravotní péče. Spoluúčast pacientů by vedla k uvědomění si ceny zdraví, a tak se dá předpokládat, že by občané byli motivováni starat se o své zdraví. Dalším pozitivním efektem spoluúčasti pacientů je i snížení míry nadužívání zdravotní péče. Odhaduje se, že 10-20% zdravotní péče je nadužíváno. Mimo financování je systém zdravotnictví, a tak i konkrétní zdravotnické zařízení, ovlivňován i řadou jiných faktorů, jako například demografickou situací, politickým systémem a stavem legislativy. Demografická situace v České republice je velmi podobná demografické situaci v okolních zemích. Vyznačuje se zejména stárnoucí populací a přílivem přistěhovalců z východních zemí. Politický systém je dle mého názoru stabilní. Legislativní oblast vyžaduje zásadní změny. Ty by se měly zejména týkat úhrady zdravotní péče z veřejného zdravotního pojištění. Současný stav nedefinuje rozsah zdravotní péče hrazené z veřejného zdravotního pojištění. Tuto situaci by vyřešilo stanovení rozsahu veřejné služby ve zdravotnictví. Zároveň by se otevřel prostor pro soukromé zdroje financí. Výrazným nedostatkem stávající legislativy je i právní forma příspěvkových organizací, ve kterých je většina velkých nemocnicí provozována. To je i případ FN Brno. Dalším problémem, který by měl být řešen je vztah lékařských fakult a fakultních nemocnic. Komplikace vznikají zejména v pracovněprávních vztazích přednostů klinik. Menšinové zdravotní pracovní úvazky přednostů mají za následek jejich nepřítomnost na klinice, za kterou nesou odpovědnost. Tuto situaci by mělo vyřešit vytvoření univerzitních nemocnic. Univerzitní nemocnice by byla součástí lékařské fakulty pod resortem ministerstva školství. Přednostové klinik by byli přímo podřízení řediteli nemocnice jak v oblasti poskytování zdravotní péče, tak i v oblasti vzdělávání. Ředitel univerzitní nemocnice by byl podřízen děkanovi fakulty. Zároveň se změnou právní formy fakultních nemocnic by měl být změněn i systém jejich financování. Fakultní nemocnice jsou financovány systémem paušálních plateb, který předpokládá, že většina finančních nákladů nemocnic je fixních. Ve spojení se zvyšující se hodnotou výkonového bodu tak má za následek faktické omezení poskytované zdravotní péče a prodlužování čekacích lhůt na neakutní operace. Jedním z možných způsobů řešení tohoto problému je používání systému DRG, který bude s největší pravděpodobností v České republice zaveden. Pro FN Brno to ovšem znamená nutnost výrazného zlepšení kvality výkaznictví, které bylo ve SWOT analýze identifikováno jako její největší slabina.

Abstract:

The way the whole system of healthcare is organised is very important for the organization providing healthcare service. Each state manages its healthcare system in a different way. The main differences can be found especially in the area of financing the healthcare. The healthcare system in the United States of America is based mainly on private sources of financing. Only approximately 40% of financial costs comes from public sources. On the other hand most european countries use mainly public sources for financing healthcare systems. In the Czech Republic the system of financing healthcare is based on public health insurance. However it is becoming obvious that permanently developing medicine will demand more and more financial sources. One of the possible solutions to this problem is the implementation of patients participation in financing the healthcare. This would be accomplished by direct payments or private health insurance or by a combination of these two sources. Not only would there be more money in the healthcare system, but also a decrease in consumption of the healthcare is expected. It is possible to presume that patients participation would result in them realizing the price of health and that patients would be motivated to take care about their health. Another positive effect is a decrease in the healthcare overuse rate. It is estimated, that approximately 10-20% of healthcare is overused. Apart from financing healthcare, the whole healthcare system and also the healthcare organization are influenced by various other factors. For example by the demographic situation, political situation and by the condition of the legislation. Demographic situation in the Czech Republic is similiar to the demographic situation in neighbouring countries. It can be characterized by an elderly population and high numbers of immigrants from eastern counries. In my opinion the political system of the Czech Republic is stable. The legislative branch requires crucial changes which should especially concern financing healthcare from public health insurance. Current law does not define the range of the healthcare covered by public health insurance. This situation could be solved by the implementation of the public service concept into the healthcare system. This would also enable private sources of financing healthcare to enter the system. The legal form of an allowance organization, in which most large hospitals are established, represents another serious problem of the current legislation. That is also the case of the FN Brno. Problems arising from the relationship of FN Brno and LFMU should also be solved. Complications arise mainly from employment law regulation of the chiefs of the clinics. Their minority health workloads cause their absence from the clinic for which they are responsible. The concept of a university hospital should help to solve this problem. The university hospital would be a part of a medical faculty under the authority of the ministry of education. Chiefs of clinics would be directly subordinated to the hospital director both in the healthcare area and in the education area. The hospital director would be subordinated to the faculty dean. Simultaneously with the change of the teaching hospitals legal form the system of their financing should be changed. Teaching hospitals are financed by a fixed payment tariff, which presumes that most hospital expenses are fixed. Along with the increasing value of perfomance points the fixed payment tariff causes actual restriction of healthcare provision and extension of waiting periods for non-acute surgeries. DRG system, which is likely to be implemented in the Czech Republic, represents a possible solution to this problem. For FN Brno implementation of the DRG system means a necessity to improve the quality of its recording which was found to be very low in the SWOT analysis.
