Abstract:

Práce se zabývá využitím terapeutického monitorování léčiv v klinické praxi, konkrétně v psychiatrii. Sesbíraná data byla získána z Žádanek o toxikologické vyšetření, které byly součástí zdravotnické dokumentace pacientů hospitalizovaných na Psychiatrické klinice v letech 2019 a 2020. Na základě vztahu denní dávky a plazmatické koncentrace léčiva vyjádřené jako poměr (C/D) jsem sledoval interindividuální variabilitu farmakokinetiky olanzapinu, klozapinu a risperidonu, které jsem statis-ticky popsal a graficky zpracoval. Plazmatické koncentrace léčiv byly interpretovány i s odkazem na referenční rozmezí vztažené k dávce, které může upozornit na přítomnost případných farmakokinetických odlišností, které jsou u části populace pacientů očekávané. Dále jsem srovnal poměr C/D u podskupin žen a mužů a nekuřáků a kuřáků, protože pohlaví a kouření mohou významně ovlivňovat plazmatickou koncentraci sledovaných antipsychotik v důsledku rozdílné aktivity izoenzymů CYP450, které metabolizují sledovaná antipsychotika. Na interindividuální variabilitu farmakokinetiky antipsychotik měly významný vliv kouření u skupiny pacientů, kterým byl podávám klozapin a pohlaví u skupiny pacientů léčených olanzapinem. TDM jako nástroj moderní medicíny umožňuje personalizovat psychofarmakoterapii při využití dosavadních poznatků v oblasti farmakodynamiky a farmakokinetiky a tím zvýšit její účinnost a bezpečnost, což může být pro pacienty benefitem.

Abstract:

The work deals with the use of therapeutic drug monitoring in clinical practice, specifically in psychiatry. The collected data were obtained from the documents called „Requests for toxicological examination“, which were part of the medical records of patients hospitalized at the Psychiatric Clinic in 2019 and 2020. Based on the relationship between the daily dose and the plasma concentration of the drug expressed as a ratio (C/D), I monitored the interindividual variability of the pharma-cokinetics of olanzapine, clozapine and risperidone, which I described statistically and graphically processed. Plasma drug concentrations we-re also interpreted in reference to a dose-related reference range, which may indicate the presence of potential pharmacokinetic differen-ces that are expected in a subset of the patient population. Furthermore, I compared the C/D ratio in subgroups of women and men and non-smokers and smokers, because gender and smoking can significantly influence the plasma concentration of the studied antipsychotics due to the different activity of the CYP450 isoenzymes that metabolize the stu-died antipsychotics. The interindividual variability of the pharmacoki-netics of antipsychotics was significantly influenced by smoking in the group of patients treated with clozapine and gender in the group of pa-tients treated with olanzapine. TDM, as a tool of modern medicine, ma-kes it possible to personalize psychopharmacotherapy using current knowledge in the field of pharmacodynamics and pharmacokinetics and thereby increase its effectiveness and safety, which can be a benefit for patients.
