Risk factors of lifestyle and cardiovascular disease

Abstract:

Kardiovaskulární onemocnění (KVO) jsou nejčastější příčinou úmrtí a výrazným původcem invalidity a nemocnosti nejen v České republice, ale i ve většině vyspělých zemí. Výrazný vliv na vznik těchto onemocnění má nevhodný životní styl, proto je důležité věnovat pozornost právě těm faktorům, které lze účinně prevencí ovlivnit. Diplomová práce se zabývá tématem kardiovaskulárních onemocnění a faktory jejich vzniku. V teoretické části jsou uvedeny a popsány nejvýznamnější onemocnění srdce a cév, rizikové faktory podílející se na jejich vzniku a druhy a možnosti prevence. Praktická část se zabývá analýzou dat, získaných v dotazníkovém šetření. Cílem praktické části práce bylo zjistit rozdíly v oblastech životního stylu mezi skupinami osob s výskytem a bez výskytu nemocí oběhové soustavy. Sběr dat probíhal anonymně formou sestaveného dotazníku, který byl distribuován v ordinaci praktického lékaře. Dotazník byl určen pro muže a ženy ve věku od 30 let, s cílem získat 50 dotazníků od osob s onemocněním a 50 od osob bez něj. Bylo distribuováno 119 dotazníků s návratností 100%, přičemž osob s onemocněním se podařilo oslovit pouze 47. Data byla vložena do programu MS Excel s použitím základní popisné statistiky. Pro testování rozdílů mezi soubory byl použit program Stata verze 13, s použitím chí-kvadrát testu, v případě nesplnění podmínky pro jeho použití byl použit Fisherův exaktní test a Mann-Whitneyův pořadový test. Celkový soubor tvořilo 119 osob, z toho 56 mužů a 63 žen, průměrný věk respondentů byl 58 let. Přítomnost kardiovaskulárních onemocnění se ve sledovaném souboru prokázala častěji u žen (60%), u mužů ze 40%. U nemocných se ve většině případů vyskytovala hypertenze (87%) a dále ve 23 % infarkt myokardu. Skupina zdravých a nemocných KVO se lišila ve vzdělání a rodinném stavu, ve skupině nemocných KVO byl větší počet osob se základním vzděláním (p=0,001) a větší počet vdov a vdovců (p= 0,001). V souboru s onemocněním KVO bylo také více ex-kuřáků (p=0,025) a větší počet osob, které přestaly konzumovat alkohol (p<0,001). Dalším rizikovým faktorem, který se vyskytoval častěji u nemocných osob, byly vyšší hodnoty BMI (p=0,029) a častější výskyt diabetu.

Abstract:

Cardiovascular disease is the most frequent cause of death and significantly contributes to invalidity and many illnesses of people in most of developed countries in the world including the Czech Republic. The main factor of these diseases is an unhealthy lifestyle, therefore awareness of this factor is crucial to the prevention of the cardiovascular diseases. This master thesis deal with the problem of cardiovascular diseases and factors that contributes to them. The former is rather theoretical and highlights the most common heart and vascular diseases, risk factors that cause them and forms of prevention. The latter analyses data from my research. The aim of this thesis is to find out what attitudes to healthy lifestyle people with and without cardiovascular diseases have. The data for the research was obtained from anonymous questionnaires that were distributed to people at a general practitioner?s surgery. The questionnaire was distributed to men and women at the minimum age of 30 with the aim of 50 answers from people who have a cardiovascular disease and 50 from people who do not have any. There were 119 questionnaires distributed and the response rate was 100%, however only 47 questionnaires were filled in by people who have a cardiovascular disease. The data were analyzed in MS Excel using descriptive statistics. The differences between answers from the two groups were analyzed in the PC app Stata version 13. A chi-square test was performed. In cases where the chi-square test was not able to use, Fisher's exact test and Mann?Whitney U test were performed. The sample size was 119 respondents ? 56 men and 63 women at the average age of 58. 60% of respondents who suffered from a cardiovascular disease were female and 40% of them were male. The most occurred problem of those was hypertension (87% occurrence) and 23% of respondents stated heart attack. The main differences between the group of healthy people and those who had cardiovascular disease were their level education and marital status. The latter group had more individuals with only primary education (p value=0.001) and there were more widowed people compare to the group of healthy people (p value=0.001). Therefore, in the latter group there was higher number of former smokers (p value=0.025) and people who used to drink alcohol (p value < 0,001). Another risk factor that was frequent within the group of people with cardiovascular disease is higher levels of BMI (p value=0.029) and higher occurrence of diabetes (p value= 0.001).
