Abstract:

Úvod: Bakalářská práce se zabývá fyzioterapií u pacientky s HV (hallux valgus) obohacenou o nácvik přechodu na barefoot obuv pro podporu rekonvalescence. Cíl práce: Cílem práce je přispět k řešení problematiky fyzioterapie u pacientky po operaci HV. Metodika: Před první terapií jsem provedla vstupní kineziologický rozbor. Na základě vstupního vyšetření jsem vytvořila krátkodobý rehabilitační plán. Terapií bylo 8 v rozsahu 50- 110 minut na jedno sezení. Součástí terapie byl nácvik přechodu na barefoot obuv a její postupné zařazování do dennodenního využívání. Po poslední terapii jsem provedla výstupní vyšetření. Porovnáním vstupních a výstupních hodnot, jsem zjistila účinnost terapie a poté jsem vytvořila dlouhodobý rehabilitační plán. Výsledky: Terapie pomohla u levého palce se snížením bolestivosti dle škály VAS (visual analogue scale) (ze 7 na 2), zvýšením pasivního rozsahu pohybu v MTP (metatarsophalangealní) kloubu levého palce [FL (flexe) z 40 st. na 45 st., EX (extenze) st. z 25 st. na 55 st., ABD (abdukce) z -7 st. na +8 st.] i aktivního rozsahu pohybu (FL z 25 st. na 35 st., EX z 25 st. na 50 st., ABD z -10 st. na 0 st.) a svalové síly (FL nebyla vyšetřena pro bolest poté 4 , EX ze 4 na 5, ABD z 3 bez plného RP na 3 s plným RP) . Zlepšila se stabilita na IMOOVE (s držením před 4,7 na 5,4, bez držení 3,5 na 3,7) odvíjení chodidla a celkový cyklus chůze. Závěry: Pooperační terapie pomohla pacientce subjektivně ve snížení bolestivosti, a tím, že ji palec již neomezuje při běžných činnostech. Objektivně se zvětšil RP v 1. MTP kloubu levého palce do FL, EX a ABD, zvýšila se stabilita na IMOOVE a zlepšil se cyklus chůze. Díky těmto výsledkům mohu soudit, že kombinace fyzioterapie a barefoot obuvi se dá využít u pacientů po operaci deformity HV.

Abstract:

ABSTRACT Introduction: This bachelor's thesis examines physical therapy for a patient with hallux valgus (HV), supplemented by training in the transition to barefoot shoes to support recovery. Objective: The objective of this thesis is to contribute to addressing the challenges of physical therapy for patients following HV surgery. Methodology: Before the first therapy session, I performed an initial kinesiological analysis. Based on the initial examination, I created a short-term rehabilitation plan. The therapy consisted of 8 sessions ranging from 50 to 110 minutes per session. Part of the therapy involved training in the transition to barefoot shoes and their gradual incorporation into daily use. After the final therapy session, I performed a follow-up examination. By comparing the initial and final values, I determined the effectiveness of the therapy and then created a long-term rehabilitation plan. Results: The therapy helped reduce pain in the left big toe according to the VAS (visual analog scale) (from 7 to 2), increased passive range of motion in the MTP (metatarsophalangeal) joint of the left big toe [FL (flexion) from 40° to 45°, EX (extension) from 25° to 55°, ABD (abduction) from -7° to +8°] as well as active range of motion (FL from 25° to 35°, EX from 25° to 50°, ABD from -10° to 0°) and muscle strength (FL was not assessed due to pain; EX from 4 to 5, ABD from 3 without full RP to 3 with full RP). Stability on IMOOVE improved (with support from 4.7 to 5.4, without support from 3.5 to 3.7), as did foot roll-out and the overall gait cycle. Conclusions: Postoperative therapy helped the patient subjectively by reducing pain and allowing her to perform daily activities without being limited by her big toe. Objectively, the range of motion (ROM) in the first metatarsophalangeal (MTP) joint of the left big toe increased in flexion (FL), extension (EX), and abduction (ABD); stability on the IMOOVE test improved; and the gait cycle improved. Based on these results, I conclude that the combination of physical therapy and barefoot shoes can be beneficial for patients following hallux valgus surgery.
