Risk factors for cardiovascular diseases in women

Abstract:

Úvod: Kardiovaskulární onemocnění (KVO) jsou hlavní příčinou úmrtí nejen v České republice, ale také ve většině vyspělých zemí. Jedním z hlavních ovlivnitelných rizikových faktorů je nevhodný životní styl. Cílem praktické části bylo zjistit výskyt vybraných rizikových faktorů mezi skupinami žen s KVO a bez tohoto onemocnění. Metodika: Data byla sbírána pomocí anonymního dotazníkového šetření. Dotazník byl vyhotoven ve dvou variantách, pro ženy s KVO (případy) a ženy bez KVO (kontroly). Dotazník byl určen ženám ve věku 55 a více let. Data byla získávaná v ordinacích praktických lékařů a také v Městské nemocnici Ostrava. Údaje z dotazníku byly vloženy do programu MS Excel. Pro zpracování dat byla použita základní popisná statistika a chí-kvadrát test. Bylo vypočteno hrubé OR (Odds Ratio) s 95% intervaly spolehlivosti a byl sestaven model, který byl vyhodnocen pomocí logistické regrese. Statistické testy byly vyhodnoceny na hladině významnosti 5 %. Pro zpracování byl použit program Stata verze 13. Výsledky: Celkem bylo vyplněno 224 dotazníků, z toho 94 žen s KVO a 130 kontrol. Průměrný věk žen byl 59,5 let (SD = 3,51). U nemocných žen se v 67 % vyskytovala hypertenze. U všech rizikových faktorů byl zjištěn vyšší výskyt u případů. V kouření, konzumaci alkoholu a sportovní aktivitě nebyl rozdíl statisticky významný. Ženy s KVO se stravovaly významně více nepravidelně (p = 0,034) (OR = 1,78), byly také významně více vystaveny stresu v pracovním (p = 0,008) i v osobním životě (p = 0,001) (OR = 5,5). Dalším rizikovým faktorem, který se významně více (p = 0,0003) vyskytoval u případů, byly vyšší hodnoty BMI. Také v rodinné anamnéze se vyskytovala významně více hypertenze (p = 0,015) a jiné KVO (p = 0,004) u případů proti kontrolám. Závěr: Z výsledků dotazníkového šetření plyne, že ženy s kardiovaskulárním onemocněním měly vyšší výskyt všech zkoumaných rizikových faktorů než ženy bez tohoto onemocnění. Statisticky významný rozdíl byl zjištěn u stravování, stresu, BMI a rodinné anamnézy.

Abstract:

Introduction: Cardiovascular diseases (CVD) are the main cause of the death not only in the Czech Republic but also in most of the developed countries. One of the main influenced risk factors is inappropriate lifestyle. The aim of the practical part was to find out the occurrence of the selected risk factors between groups of women with CVD and without the disease. Methods: The data were collected using an anonymous questionnaire. The questionnaire was prepared in two versions, for women with CVD (cases) and for women without CVD (controls). The questionnaire was designed for women aged 55 years or more, practitioners cooperated to obtain data and data were also collected in City Hospital Ostrava. Data from the questionnaire has been entered into Microsoft Excel program. The basic descriptive statistics and chi quadrate test were used for data processing. Roughly OR (Odds Ratio) was calculated with 95% confidence interval and it was built a model that was evaluated using logistic regression. Statistical tests were evaluated at a significance level of 5 %. For processing of data was used Stata version 13th. Results: A total of 224 questionnaires were filled, of which 94 women with CVD and 130 controls. The average age of respondents was 59,5 years (SD = 3,51). The presence of hypertension in female patients was 67 %. All risk factors were found to be higher in the case group. In the smoking, alcohol consumption and sporting activity, groups did not statistically significant differ. Women with CVD were eating more irregularly (p = 0,034) (OR = 1,78). They were also significantly more exposed to stress in work (p = 0,008) and in their personal life (p = 0,001) (OR = 5,5). Another risk factor which significantly occurred more often (p = 0,0003) in a group of cases was higher BMI values. Also, in family medical history hypertension (p = 0,015) and other CVD (p = 0,004) occurred significantly more among cases against controls. Conclusion: The results of the survey showed that women with cardiovascular disease had a higher presence all examined risk factors than woman who were without CVD. Statistically significant differences were found in diet, stress, BMI and medical history.
