Assessment of cranial gunshot wounds through postmortem CT

Abstract:

This work deals with the evaluation of cranial gunshot wounds on the post-mortem CT (PMCT) images. With the help cranial gunshot wounds on the PMCT records there’s possible to evaluate the direction and trajectory of the projectile in the skull, the damage extent of gunshot wound, estimate the firing distance, estimate the speed, type and calibre of the ammunition and estimate the type of weapon used, or estimate the motive of the shooting according to the position of the bullet holes on the skull. The aim of the practical part of the work was the description and analysis of cranial gunshot wounds using PMCT records created as a part of the forensic medical examination in Hradec Králové. The type of gunshot wound was subsequently determined based on selected characteristic features of cranial gunshot wounds on PMCT records in 3D Slicer program. Imaging methods were used to visualize PMCT images of cranial gunshot wounds; Volume rendering in combination with sections and a 3D model. Based on the assessments of anthropologists, the visualization method Volume rendering with sections is more time-consuming compared to the assessment on 3D models. However, when taking into account the time of preparation of the given outputs, the time requirement of the imaging methods is balanced. However, the time-consuming of the assessment also depends on the type of gunshot wound, while the type of gunshot wound with two gunshot holes with a harder-to-detect entrance hole in the oral cavity, proved to be more time-consuming. Based on the answers detected by anthropologists, the 3D model was evaluated as the more reliable of the two imaging methods for a difficult identifiable gunshot wounds on the PMCT recording. On the contrary, for a clearly identifiable gunshot wound on the PMCT recording, volume rendering in combination with sections turned out to be a more reliable method. Finally, there was noted a significant degree of dependence between the largest diameter of the entrance bullet holes and the thickness of the skull diploe.

Abstract:

Tato práce se zabývá hodnocením střelných poranění lebky na post-mortem CT (PMCT) záznamech. Pomocí PMCT záznamů střelných poranění lebky je tak možné hodnotit směr a trajektorii projektilu v lebce, rozsah poškození střelného poranění, odhad vzdálenosti střelby, odhad rychlosti, typu a kalibru munice a odhad typu použité zbraně či odhad motivu střelby podle polohy střelných otvorů na lebce. Cílem praktické části práce byl popis a analýza střelných poranění lebky pomocí PMCT záznamů vytvořených v rámci soudně-lékařského vyšetření v Hradci Králové. Na základě vybraných charakteristických znaků střelných poranění lebky na PMCT záznamech v programu 3D Slicer se následně určil typ střelného poranění. Pro vizualizaci PMCT záznamů střelných poranění lebky byly použity tyto zobrazovací metody; objemové vykreslení v kombinaci s řezy a 3D model. Na základě hodnocení antropologů je zobrazovací metoda objemového vykreslení s řezy časově náročnější v porovnání s hodnocením na 3D modelech. Avšak při započtení i času přípravy daných výstupů je časová náročnost zobrazovacích metod vyrovnaná. Časová náročnost hodnocení také závisí na typu střelného poranění, přičemž jako časově náročnější se ukázal typ střelného poranění se dvěma střelnými otvory s obtížněji detekovatelným vstupním otvorem v dutině ústní. Na základě odpovědí a detekovaných nálezů antropology ze dvou uvedených zobrazovacích metod, 3D modely byly vyhodnoceny jako spolehlivější metoda pro těžko identifikovatelná střelná poranění na PMCT záznamu. Naopak pro jasně identifikovatelné střelné poranění na PMCT záznamu se prokázala jako spolehlivější metoda objemového vykreslení v kombinaci s řezy. Nakonec byl zaznamenán význačný stupeň závislosti mezi největším průměrem vstupních střelných otvorů a tloušťkou diploe lebky.
