Abstract:

Úvod: Bolest zad při vykonávání práce dentální hygienistky může být ovlivněna dlouhodobým působením negativních faktorů. Významným faktorem je zaujímání nesprávných nebo statických poloh během práce. Nejčastěji postihuje bolest zad dentální hygienistky v oblasti krční (30,6 %) a lumbosakrální (23,3 %). Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie a ergonomie u dentální hygienistky. Metodika: Metodika práce obsahuje kazuistiku ženy ve věku 46 let, která pracuje patnáct let jako dentální hygienistka a často trpí během práce bolestí zad v krční a bederní oblasti. Byl proveden vstupní kineziologický rozbor, na základě kterého byl navržen krátkodobý fyzioterapeutický plán. Následně proběhlo deset terapií v období od 4. ledna 2024 do 9. března 2024, přičemž délka jedné terapie se pohybovala v rozmezí hodiny a hodiny a půl. Tři terapie probíhaly v pracovním prostředí a sedm v domácím prostředí. Během terapií byly využívány metody akrální koaktivační terapie, manuální terapie, spirální stabilizace páteře, dynamická neuromuskulární stabilizace, jógové ásany, metody Ludmily Mojžíšové, Brügger koncept, míčkování a proprioceptivní neuromuskulární facilitace. Na závěr byl proveden výstupní kineziologický rozbor a stanoven dlouhodobý fyzioterapeutický plán. Výsledky: Po deseti týdenní nastavené intervenci došlo během práce v ordinaci dentální hygienistky ke snížení frekvence a intenzity bolestí v krční a bederní oblasti a také se zlepšila postura těla. Celkově se u pacientky zlepšilo postavení hlavy a pažních pletenců. Dále došlo ke zmenšení brániční insuficience, výraznému prohloubení bráničního dýchání a zlepšení dechového stereotypu. Dynamika páteře se zlepšila při Stiborově testu. Závěry: Ke snížení bolesti došlo v oblasti krční a bederní páteře, avšak pro udržení pozitivních výsledků je důležité pokračovat ve fyzioterapeutickém cvičení a dodržování ergonomických úprav spolu s pravidelnými kontrolami.

Abstract:

Introduction: Back pain when working as a dental hygienist can be affected by long-term exposure to negative factors. A significant factor is taking incorrect or static positions during work. Dental hygienists are most often affected by back pain in the cervical (30.6%) and lumbosacral (23.3%) regions. The objective: The aim of the bachelor thesis is to contribute to the solution of the problem of physiotherapy and ergonomics in dental hygienist. Methodology: The methodology of the work includes a case study of a 46year-old woman who has been working as a dental hygienist for fifteen years and often suffers from back pain in the cervical and lumbar region during work. An initial kinesiological analysis was performed, based on which a short-term physiotherapy plan was proposed. Subsequently, ten therapy sessions were conducted between 4 January 2024 and 9 March 2024, with the length of one therapy session ranging between an hour and an hour and a half. Three therapies were conducted in a work setting and seven in a home setting. During the therapies, acral coactivation therapy, manual therapy, spiral spinal stabilization, dynamic neuromuscular stabilization, yoga asanas, Ludmila Mojžíšová methods, Brügger concept, balling and proprioceptive neuromuscular facilitation were used. Finally, an outcome kinesiological analysis was performed and a long-term physiotherapy plan was established. Results: After the ten-week intervention, the frequency and intensity of pain in the neck and lumbar region was reduced during work in the dental hygienist's office, and posture improved. Overall, the patient's head and brachial plexus posture improved. Furthermore, there was a reduction in diaphragmatic insufficiency, a significant deepening of diaphragmatic breathing and an improvement in breathing stereotype. Spinal dynamics improved on the Stiboro. Conclusions: Pain reduction occurred in the cervical and lumbar spine, but continued physiotherapy exercises and ergonomic adjustments along with regular check-ups are important to maintain positive results.
