Abstract:

Introduction: Depressive symptoms are occured in 31% patients with diabetes mellitus. In patients with diabetes mellitus is risk for development of depression two times higher in compare with general population. We know some risk factors – demographic (female gender, younger age, lower education, poorness), clinical (relevance of diabetes, disorder length, complications of diabetes, increased leveles of glycosyled haemoglobin HbA1c) behavioural (smoking, obesity), which more or less related to occur depression in patients with diabetes. Among the endocrine and neurotransmitter changes include decreased concentrations of catecholamines, mainly 5-HT, stimulation the production of glucocorticoides, growth factor and glucagon, which act counter to the hypoglycaemic action of insulin. Increased cortisol levels are observed equally in patients with diabetes and depression, like impaired glucose tolerance and the development of insuline resistance. Common occurrence of these two disorders is not a random phenomenon, which gives account of their possible relationship. The depression arises as a direct result of neurochemical changes associated with diabetes or depression is result of psychosocial factors related to disease/treatment or on other hand if depression is a independent risk factor for diabetes. AIM: The main objective of this work was to assess the safety of antidepressant treatment in terms of the impact on selected glucose and lipid metabolic parameters in the group of SSRI´s and SNRI´s preparations in patients with major depressive disorder in natural conditions. A side goal was to assess the effectiveness of treatment for depression and compare the two groups to each other antidepressants. Methods: A follow-up included a total of 74 patients with a diagnosis of a depressive phase or recurrent major depressive disorder who were the clinical discretion of the physician divided into two subgroups. One subgroup was treated with SNRI´s (studied group), the second group was treated with SSRI´s (control group) in a flexible dosing. Every patient was performed before treatment, at the third and sixth month of treatment, laboratory tests to evaluate the glycemic and lipid parameters and every patient also underwent assessment of psychopathology using the MADRS scales, Zung questionnaire depression and Beck anxiety questionnaire. Results: In the group studied SNRI´s occurred during and after treatment to no significant increase in blood glucose (oGTT) of 5.16 / 7.29 to 5.26 / 7.51 (ns), HbA1c decreased from 5.54 to 5.29 ( ns). LDL cholesterol increased from 3.15 to 3.18 (ns), HDL cholesterol increased from 1.31 to 1.38 (ns). Differences in levels of total cholesterol in the three follow-up were not significant (5.28 vs. 5.23, ns), but the trend over a longer follow-up assumes significance because post hoc tests showed a significant difference in the decrease of total cholesterol between the first and second follow-up. Triglycerides increased from 1.81 to 1.92 (n. s.). BMI in the group of patients with SNRI preparations are no significantly changed from 27.03 to 27.35 (ns). MADRS total score significantly decreased from 36.36 to 11.56 (p <0.001) in the SDS index Zung questionnaire depression significantly decreased from 66.95 to 51.26 and Beck questionnaire scores in anxiety significantly decreased from 23.72 to 15 , 92 In the control SSRI group differences in blood glucose levels (oGTT) during three follow-up were not significant (4.7 / 5.1 vs 6.33 / 6.33, ns), but the trend over a longer follow-up assumes significance because post hoc tests showed significant difference in the increase of blood glucose (oGTT) between the second and third follow-up. HbA1c decreased from 5.22 to 5.18 (n. s.). Differences in levels of total cholesterol were not significant (5.23 vs 5.4), but the trend over a longer follow-up assumes significance because post hoc tests showed a significant increase in total cholesterol levels between the second and third follow-up. Triglycerides decreased from 1.49 to 1.36 (n. s.). BMI measurements in the group of patients with SSRI´s were significantly increased from 25.13 to 25.98 (t = -3.68 **). MADRS total score significantly decreased from 37.83 to 12.23 (p = 0.000), SDS index within Zung questionnaire depression significantly decreased from 67.63 to 49.71 and Beck questionnaire scores in anxiety significantly decreased from 25.09 to 14.6. Conclusion: In our work, we were able to demonstrate that treatment of patients with depression type SNRI antidepressants in terms of impact on selected glycemic and lipid parameters is safe and its safety is comparable to the SSRIs and also is a comparatively efficient. This conclusion, however, should be validated in a larger number of patients and over a longer period of time can be set with stricter requirements, psychoeducation and education on diet and lifestyle.

Abstract:

Úvod: Klinicky signifikantné depresívne symptómy sa vyskytujú približne u 31% pacientov s diabetom mellitus (DM). Pri DM je riziko vzniku depresívnej poruchy približne 2x vyššie ako v bežnej populácii. Poznáme niekoľko rizikových faktorov: demografické (ženské pohlavie, mladší vek, nižšie vzdelanie, chudoba), klinické (závažnosť DM, dĺžka ochorenia, komplikácie diabetu, vysoké hodnoty glykozylovaného HbA1c) aj behaviorálne (fajčenie, obezita), ktoré viac či menej súvisia s výskytom depresie u pacientov s DM. Medzi endokrinné a neurotransmiterové zmeny patrí znížená koncentrácia katecholamínov, predovšetkým sérotonínu, stimulácia produkcie glukokortikoidov, rastového hormónu a glukagónu, ktoré pôsobia kontraregulačne proti hypoglykemickému účinku inzulínu. Zvýšené hladiny kortizolu sa pozorujú rovnako u pacientov s DM aj depresiou, podobne porucha glukózovej tolerancie a vznik inzulínovej rezistencie. Spoločný výskyt uvedených dvoch porúch nie je náhodným javom, čo vyvoláva úvahy o ich možnom vzťahu, pričom depresia môže vzniknúť ako priamy dôsledok neurochemických zmien, ktoré súvisia s DM alebo je depresia dôsledkom psychosociálnych faktorov, ktoré súvisia s ochorením/liečbou diabetu, či na druhej strane depresia je nezávislým rizikovým faktorom pre vznik DM. Cieľ: Hlavným cieľom práce bolo posúdiť bezpečnosť antidepresívnej liečby v zmysle vplyvu na vybrané glykemické a lipidové metabolické parametre v skupine SNRI a SSRI preparátov u pacientov s depresívnou poruchou v prirodzených podmienkach. Vedľajším cieľom práce bolo posúdiť účinnosť antidepresívnej liečby a porovnať navzájom obe skupiny antidepresív. Metodika: Do sledovania bolo zaradených celkovo 74 pacientov s diagnózou depresívnej fázy či rekurentnej depresívnej poruchy, ktorí boli podľa klinického uváženia lekára rozdelení do dvoch podskupín. Jedna podskupina bola liečená antidepresívami typu SNRI (sledovaná skupina), druhá skupina bola liečená preparátmi typu SSRI (kontrolná skupina) vo flexibilnom dávkovaní. U každého pacienta bol realizovaný pred začiatkom liečby, v treťom a šiestom mesiaci liečby laboratórny odber za účelom zhodnotenia glykemických a lipidových parametrov a taktiež sa každý pacient podrobil zhodnoteniu psychopatológie pomocou škál MADRS, Zungovho dotazníka depresie a Beckovho dotazníka úzkosti. Výsledky: V sledovanej SNRI skupine došlo počas liečby a po liečbe k nesignifikantnému nárastu glykémie (oGTT) z 5,16/ 7,29 na 5,26/ 7,51 (n. s.), HbA1c klesol z 5,54 na 5,29 (n. s.). LDL cholesterol stúpol z 3,15 na 3,18 (n. s.), HDL cholesterol sa zvýšil z 1,31 na 1,38 (n. s.). Rozdiely v hodnotách celkového cholesterolu počas troch sledovaní neboli signifikantné (5,28 vs 5,23, n. s.), ale trend v dlhšom sledovaní predpokladá signifikanciu, nakoľko post hoc testy ukázali signifikantný rozdiel v poklese celkového cholesterolu medzi prvým a druhým sledovaním. Triglyceridy sa zvýšili z 1,81 na 1,92 (n. s.). Hodnoty BMI v skupine pacientov s SNRI preparátmi sa nesignifikantne zmenili z 27,03 na 27,35 (n. s.). Celkové skóre MADRS signifikantne kleslo z 36,36 na 11,56 (p<0,001), SDS index v rámci Zungovho dotazníka depresie signifikantne klesol zo 66,95 na 51,26 a skóre v Beckovom dotazníku úzkosti signifikantne kleslo z 23,72 na 15,92. V kontrolnej SSRI skupine rozdiely v hodnotách glykémie (oGTT) počas troch sledovaní neboli signifikantné (4,7/ 6,33 vs 5,1/ 6,33, n. s.), ale trend v dlhšom sledovaní predpokladá signifikanciu, nakoľko post hoc testy ukázali signifikantný rozdiel v náraste glykémie (oGTT) medzi druhým a tretím sledovaním. Triglyceridy klesli z 1,49 na 1,36 (n. s.). Hodnoty BMI v jednotlivých meraniach pre skupinu pacientov s SSRI preparátmi signifikantne stúpli z 25,13 na 25,98 (t = -3,68**). Celkové skóre MADRS signifikantne kleslo z 37,83 na 12,23 (p=0,000), SDS index v rámci Zungovho dotazníka depresie signifikantne klesol zo 67,63 na 49,71 a skóre v Beckovom dotazníku úzkosti signifikantne kleslo z 25,09 na 14,6. Záver: V našej práci sa nám podarilo preukázať, že liečba pacientov s depresiou antidepresívami typu SNRI v zmysle vplyvu na vybrané glukemické a lipidové parametre je bezpečná a jej bezpečnosť je porovnateľná s antidepresívami typu SSRI a taktiež je aj porovnateľne účinná. Tento záver by mal však byť overený na väčšom počte pacientov a počas dlhšieho časového obdobia možno so striktnejšie nastavenými požiadavkami na psychoedukáciu či edukáciu o životospráve a životnom štýle.
