SELDI-TOF MS profiling of pathogenic bacteria focused on Enterobacteriaceae

Abstract:

Hmotnostní spektrometrie (MS) je v klinické mikrobiologii rutinně využívaná technika pro identifikaci bakterií. Princip identifikace MS analýzou spočívá ve vygenerování unikátního hmotnostního spektra pro každý mikroorganismus. Hmotnostní spektrum (MS) zaznamenává proteiny exprimované bakterií na základě jejich molekulové hmotnosti. Potenciální využití MS profilů pro stanovení antibiotické rezistence a biochemického fenotypu bylo předmětem této diplomové práce. Za použití SELDI-TOF MS, která je variantou MALDI-TOF MS techniky, a s použitím kovových čipů bez upraveného povrchu byly analyzovány kmeny Klebsiella pneumoniae a Escherichia coli. V případě K. pneumoniae shluková analýza identifikovala soubor píků u cefoxitinu, na jejichž základě je možné rozlišit citlivé kmeny od rezistentních. Dále byly identifikovány píky představující biomarkery pro stanovení citlivosti/rezistence k aztreonamu a cefotaximu v závislosti na jejich intenzitě. V případě E. coli byly identifikovány píky, jejichž intenzita charakterizuje citlivost/rezistenci k antibiotikům co-trimoxazolu a cefazolinu a dále píky, jejichž intenzita rozliší laktózový fenotyp. MS analýza umožňuje tuto identifikaci bez kultivace na selektivních médiích a bez kultivace s antibiotiky.

Abstract:

Mass spectrometry (MS) is in clinical microbiology technique routinely used to identify bacteria. The principle of identifying MS analysis is to generate a unique mass spectra for each microorganism. Mass spectrum (MS) records the expressed proteins of bacteria on the basis of their molecular weight. Potential use of MS profiles for the determination of antibiotic resistance and biochemical phenotypes was the subject of this thesis. Applying of SELDI-TOF MS, which is a variant of MALDI-TOF MS techniques, and using metal chips without the modified surface there were analyzed strains of Klebsiella pneumoniae and Escherichia coli. In the case of Klebsiella pneumoniae cluster analysis identified a set of peaks with cefoxitin, on the basis of which it is possible to distinguish susceptible from resistant strains. To establish sensitivity/resistance to aztreonam and cefotaxime according to their intensity, peaks representing biomarkers were identified. In the case of Escherichia coli peaks whose intensity is characterized by sensitivity / resistance to the antibiotic co-trimoxazole, and cefazolin were identified as well as peaks whose intensity distinguish lactose phenotype. MS analysis enables this identification without cultivation on the selective media and without antibiotics.
