Effect of physiotherapy in idiopathic scoliosis using the SM system

Abstract:

Úvod: Tématem mé bakalářské práce je komplexní fyzioterapie u AIS s využitím metody SM systém. Seznámením se s novými poznatky jsem se snažila souhrnně uchopit problematiku skoliózy a přiblížit možnosti jejího řešení fyzioterapeutickou intervencí. Cíl práce: Cílem této bakalářské práce je přispět k volbě účinných fyzioterapeutických metod, představit efektivní plán fyzioterapie a vytvořit přehledný soubor poznatků pro pacienty se skoliózou. Metodika: Tato část bakalářské práce obsahuje kazuistiku šestnáctileté dívky s diagnózou adolescentní idiopatické skoliózy. Vstupní kineziologické vyšetření včetně anamnézy jsem provedla za využití metody dialogu, observace, měření a testování. Měření jsem provedla za pomoci krejčovského metru, váhy a goniometru. Na základě vstupního rozboru jsem vyhotovila krátkodobý fyzioterapeutický plán, který jsem realizovala v deseti pravidelných terapiích během devíti týdnů. Kontrolní vyšetření jsem provedla přibližně v polovině fyzioterapeutického plánu. V kontrolním vyšetření jsem se zaměřila zejména na posouzení hybnosti páteře, respirační amplitudu a případné recidivující kloubní blokády. Jednotlivé terapie jsem cílila zejména na zlepšení elasticity hrudníku, hybnosti páteře, posílení trupového svalstva a stabilizaci ramenního pletence. K dosažení tohoto cíle jsem využila techniky měkkých tkání, DG, mobilizace dle Ludmily Mojžíšové a SM systém. Po ukončení rehabilitace jsem provedla výstupní kineziologický rozbor, na jehož základě jsem sestavila dlouhodobý fyzioterapeutický plán. Výsledky: Dle výstupního kineziologického rozboru se podařilo uvolnit většinu měkkých tkání, zlepšit kloubní vůli a odstranit hypertonus přetížených svalů. V důsledku toho se zlepšilo držení těla, hybnost páteře a elasticita hrudníku. Závěr: Práce pro mě byla velmi přínosná. Podařilo se mi s pacientkou splnit většinu stanovených cílů a získala jsem cenné zkušenosti, které mohu využít v praxi.

Abstract:

Introduction: The topic of my bachelor's thesis is complex physiotherapy for AIS using the SM system method. Acquainting myself with new findings, I tried to comprehensively grasp the issue of scoliosis and approach the possibilities of its solution through physiotherapeutic intervention. The objective: The aim of this bachelor thesis is to contribute to the choice of effective physiotherapy methods, to present an effective physiotherapy plan and to create a clear set of knowledge for patients with scoliosis. Methodology: This part of the bachelor's thesis contains a case study of a sixteen-year- old girl diagnosed with adolescent idiopathic scoliosis. I performed an initial kinesiological examination, including anamnesis, using the method of dialogue, observation, measurement and testing. I made the measurements with the help of a tailor's tape measure, a weighing-machine and a goniometer. Based on the initial analysis, I drew up a short-term physiotherapy plan, which I implemented in ten regular therapies over nine weeks. I performed the control examination approximately halfway through the physiotherapy plan. In the follow-up examination, I focused mainly on assessing the mobility of the spine, respiratory amplitude and possible recurrent joint blockages. The individual therapies were mainly aimed at improving the elasticity of the chest, mobility of the spine, strengthening the trunk muscles and stabilizing the shoulder girdle. To achieve this goal, I used soft tissue techniques, DG, mobilization according to Ludmila Mojžíšová and the SM system. After completing the rehabilitation, I performed an output kinesiological analysis, based on which I compiled a long-term physiotherapy plan. Results: According to the final kinesiology analysis, it was possible to release most of the soft tissues, improve the joint clearance and remove the hypertonus of the overloaded muscles. As a result, posture, spinal mobility and chest elasticity are improved. Conclusions: The work was very beneficial for me. I managed to meet most of the set goals with the patient and gained valuable experience that I can use in practice.
