The prognostic value of the mutational landscape in systemic anaplastic large cell lymphoma

Abstract:

Systémový anaplastický velkobuněčný lymfom (sALCL) je heterogenní onemocnění s nízkou incidencí, jehož genetické vlastnosti jsou nyní lépe prozkoumány díky implementaci vysokokapacitních genomických technologií. sALCL se dělí na dvě definované diagnostické jednotky, které se liší přítomností nebo absencí translokace genu ALK (Anaplastic Lymphoma Kinase): ALK-pozitivní ALCL a ALK-negativní ALCL. U obou subtypů je v první léčebné linii obvykle volena chemoterapie; ačkoliv pacienti s ALK-pozitivním ALCL nejprve odpovídají dobře na tento typ léčby, více než 30 % z nich relabuje. ALK-negativní ALCL se vyznačuje nejhorší prognózou s 5letým přežitím ve 40 % případů. Z toho důvodu jsou potřeba nové terapeutické možnosti i prognostické biomarkery. Zatímco genetické aberace popsané u sALCL (tj. změny počtu kopií DNA, chromosomové translokace a jednonukleotidové varianty) přinesly lepší pochopení biologie nemoci, prognostická a prediktivní hodnota somatických mutací zůstává nejasná. Předložená práce si kladla za cíl přinést nové poznatky o této problematice s využitím cíleného sekvenování DNA s vysokým pokrytím. Studovali jsme kompletní kódující oblasti 275 genů asociovaných s nádory v retrospektivním souboru 82 pacientů s sALCL (47 ALK-pozitivních a 35 ALK-negativních případů) za účelem zlepšení prognostické stratifikace a predikce agresivity onemocnění. Ukázali jsme, že mutace zasahující komponenty JAK/STAT signalizační dráhy asociují se špatnou prognózou a že zvláště mutace genu STAT3 (26 % ALK-negativních případů) jsou spojeny s kratším celkovým přežitím ve skupině ALK-negativních ALCL pacientů (poměr rizika [HR] = 4,1; 95 % CI, 1,56-10,71; p <0,002). Gen TP53 byl nejčastěji mutovaným genem u sALCL s četností 16 % pacientů (5/47 ALK-pozitivních a 8/35 ALK-negativních); mutace byly detekovány v době diagnózy i u relabujících/refrakterních pacientů. Jako první jsme navíc ukázali, že TP53 mutace jsou spojeny s kratším časem do progrese (HR = 3,3; 95 % CI, 1,59-6,87, p <0,0007). Srovnání vzorků z doby diagnózy a relapsu ukázalo, že klony s aberantním TP53 mohou akcelerovat progresi onemocnění. Data prezentovaná v této práci přispěla k podrobnější charakterizaci mutačního spektra sALCL a k identifikaci nových slibných prognostických markerů, které by mohly sloužit ke zlepšení prognostické stratifikace a péče o pacienty.

Abstract:

Systemic anaplastic large cell lymphoma (sALCL) is a rare and heterogenous disease whose genetic characterization has been improved through the implementation of high-throughput genomic technologies. sALCL encompasses two distinct clinical entities characterized by either the presence or absence of an Anaplastic Lymphoma Kinase (ALK) translocation: ALK-positive ALCL and ALK-negative ALCL. In both entities the first line treatment is chemotherapy based and although ALK-positive ALCL patients respond well to this treatment more than 30% of cases relapse. ALK-negative ALCL shows a worst prognosis with 5-year overall survival around 40%. Therefore, new therapeutic approaches and prognostic biomarkers are needed. While genetic aberrations (copy number variations, translocations, and single nucleotide variants) described in sALCL have provided a better understanding of its biology, the prognostic or predictive value of somatic mutations remains unclear. In this thesis we aim to fill this gap through the use of targeted deep DNA sequencing. We investigated the whole coding region of 275 cancer-related genes in a retrospective cohort of 82 sALCL patients (47 ALK-positive and 35 ALK-negative), with the scope to improve risk stratification and predict disease aggressiveness. We showed that mutations in JAK/STAT pathway associate with worse prognosis and in particular STAT3 mutations (26% of ALK-negative cases) associate with a shorter overall survival in ALK-negative ALCL patients (hazard ratio [HR] = 4.1; 95% CI, 1.56-10.71, p < 0.002). TP53 mutated in 16% of sALCL (5/47 ALK-positive and 8/35 ALK-negative) is the most frequently mutated gene in sALCL, both at diagnosis and in relapsed/refractory patients. Additionally, for the first time we reported that TP53 mutations associate with shorter progression free survival (HR = 3.3; 95% CI, 1.59-6.87, p < 0.0007). Moreover, the comparison of samples at diagnosis and relapse revealed that clones harboring aberrations in the TP53 may play a role in promoting disease progression. Altogether the data presented in this thesis further characterize the mutational landscape in sALCL and identify novel promising prognostic markers that could serve in the clinical improving prognostic stratification and patient management.
