Subjective difficulties of diabetics with ischemic disease of the lower limbs

Abstract:

ABSTRAKT Úvod: U diabetiků s ischemickou chorobou dolních končetin je velmi komplikované zjistit subjektivní potíže dolních končetin, kvůli výskytu přidružených onemocnění diabetes mellitus. Pro správné určení diagnózy je zapotřebí diferenciální diagnostiky a vyšetření ABI. Cíl: Hlavními cíli této práce bylo vytvoření dotazníku ke zhodnocení subjektivních potíží (bolestivosti nohou) u diabetiků s prokázanou ischemickou chorobou dolních končetin, sběr a zpracování dat u pilotního souboru pacientů navštěvujících kardiologickou ambulanci, vyhodnocení těchto dat a porovnání získaných výsledků s výsledky diabetiků s objektivně prokázanou přítomností ischemické choroby dolních končetin. Metodika: Vytvořeny dotazníky ke zjištění výskytu subjektivních potíží ischemické choroby dolních končetin u diabetiků. Využity metody ABI měření a výpočtů. Srovnány výsledky dotazníků s výsledky měření ABI. Výsledky: Pro výzkumné účely byl vytvořen dotazník, který vyplnilo 18 probandů s prokázanou ischemickou chorobou dolních končetin. Po provedení výzkumu došlo k vyhodnocení výsledků na pilotním souboru. U 16,67 % probandů nebyla uváděná bolest nohou, u zbylých 83,33 % byla uváděná bolest nohou. Dále bylo u pilotního souboru provedeno ABI měření pomocí tří metod. Výsledky ABI měření byly rozděleny do 4 skupin. Vznikly rozpory mezi naměřenými výsledky a prokázanou ischemickou chorobou dolních končetin. Z porovnání výsledků subjektivního vnímání bolesti bylo patrné, že v metodě ABI OSCI byl v 57 % výsledek měření negativní a také probandi udávali přítomnost bolesti. V 50 % tomu tak bylo u metody ABI HAP a 33 % u metody LAP. V případech pozitivních výsledků ABI byla bolest dolní končetiny udávaná u všech metod víc než v 90 % a ABI OSCI 100 %, nepřítomnost bolesti v maximu 8 % u metody ABI HAP. V případě negativního výsledku ABI byly končetiny hodnoceny pomocí klasifikace dle Fontaina a nejčastěji zařazeny do I. stádia. Příznaky tohoto stádia jsou klasifikovány jako latentní průběh bez přítomnosti klaudikačních bolestí, ABI HAP 62,5 %, ABI LAP 66,7 % a ABI OSCI 57,1 %. Závěr: Vyhodnocením dotazníků byly získány potřebné údaje o subjektivních potížích u pilotního souboru. Pomocí metody ABI byly však naměřeny hodnoty, které v některých případech nekorelovaly s odpověďmi a naopak. Bylo zjištěno, že metoda ABI měření není natolik přesná, aby u diabetiků dokázala potvrdit nebo vyloučit ischemickou chorobu dolních končetin. Srovnáním výsledků dotazníků s výsledky měření ABI u všech metod, byl zjištěn rozdíl mezi hodnotami ABI a pociťovanými bolestmi.

Abstract:

ABSTRAKT Introduction: It is very complicated to determine the subjective problems of the lower limbs in diabetic patients because of other complications associated with diabetes mellitus. For the right diagnosis are differential diagnosis and ABI examination needed. Objective: The main objectives of this seminar work are creating some questionnaires to evaluate subjective difficulties (specifically leg pain) in diabetic patients with Ischemic disease of the lower extremities, then collecting information and processing data from patients who attending cardiologic ambulances, evaluation of this data and comparing obtained information with proven presence of Ischemic disease of the lower extremities. Methods: I created the questionnaires to detection subjective problems of Ischemic disease of the lower extremities in diabetic patients. Then I also used ABI methods and I compared the results of questionnaires with ABI results. Results: The questionnaires were filled by 18 patients suffering from Ischemic disease of the lower extremities and after this research I evaluated results. 83,33 % of patients complained about leg pain, the remaining patients (16,67 %) did not mention the leg pain. The next step was to perform ABI measurements using by three methods. Then the ABI results were sort to the 4 groups. However, there were differences between the patient's results and the proven Ischemic disease of the lower extremities. During comparing the results, we found that in the ABI OSCI method, the result was negative in 57% of cases and patients complained of leg pain. In the ABI HAP method, it was 50% of cases and in the LAP method 33%. In the patients with positive ABI results was leg pain indicated in more than 90% of cases and in the ABI OSCI in 100%. In the ABI HAP method, the absence of leg pain was at most in 8% of cases. In the cases with negative results of the ABI method the lower extremities were most often evaluated by using the Fontaine classification and classified in the 1st stage. Symptoms of this stage are classified as latent course without pain (ABI HAP 62,5 %, ABI LAP 66,7 % an ABI OSCI 57,1 %). Conclusion: The necessary subjective problems in patients were obtained by evaluating the questionnaires. By using the ABI method was collected data, which disagreed with information from patients in some cases. I founded that the ABI method is not completely reliable to prove Ischemic disease of the lower extremities in patients with diabetes. By comparing the results of ABI method and the results of questionnaires there was found a difference between ABI results and pain felt by the patients.
