Carotid atherosclerosis in diabetics with lower extremity arterial disease

Abstract:

Ateroskleróza (ATS) je systémové onemocnění, které se v přítomnosti diabetu akceleruje v tepnách celého těla. Některé studie odhadují, že až 35 % pacientů trpící na aterosklerózu dolních končetin (DK) trpí zároveň významnou stenózou (> 50 %) karotických tepen, která je vysoce riziková pro cerebrovaskulární a kardiovaskulární příhody. Identifikace takovýchto pacientů je velice důležitá, neboť až 48 % mozkových příhod je způsobeno právě aterosklerózou karotických tepen. Pro identifikaci (screening) aterosklerózy DK (ICHDK) se používá metoda ankle brachial index (ABI), která je založena na měření tlaků horních a dolních končetin. Cílem této bakalářské práce bylo seznámit čtenáře s problematikou aterosklerózy karotických tepen u diabetiků s ICHDK, a to charakterizací jednotlivých onemocnění v první (teoretické) části. Tato část byla zaměřena na: rizikové faktory těchto onemocnění a jejich klinické projevy, vznik a typy aterosklerotického plátu, anatomií a patogenezí ATS karotid, která hraje důležitou roli, a také zejména diagnostikou ICHDK. Pro uvědomění problematiky je však hlavní druhá část (diskuze), ve které bylo shrnuto co nejvíce vědeckých poznatků o vztahu mezi aterosklerotickým postižením karotid a prokázanou přítomností ICHDK u diabetiků. Tyto vědecké poznatky se zaobíraly zejména: metodou ABI pro detekci ICHDK, nově zkoumaným vztahem mezi ABI a detekcí ATS karotid a charakterizací hodnot ABI. Tato práce je psána formou literární rešerše. Informace, které byly použity, pocházejí jak z odborné české, tak i odborné zahraniční literatury, která byla zejména v elektronické podobě. Zhodnocení výsledků vědeckých studií bylo velice obtížné, neboť byly ve studiích metodologické rozdíly. Přesto však bylo zjištěno, že je diabetes hlavním rizikovým faktorem aterosklerózy, a že existuje souvislost mezi ICHDK a ATS karotid u diabetických pacientů. Z výsledků studií dále vyplynulo, že je velice důležité u diabetických pacientů s ICHDK ověřit, zda pacient netrpí také ATS karotid, a/nebo v opačném případě, u pacientů, kteří prodělali cévní mozkovou příhodu (CMP) nebo mají stenózu karotických tepen (CAS) ověřit, zda netrpí také ICHDK. V této souvislosti může hrát významnou roli hodnota ABI.

Abstract:

Atherosclerosis (ATS) is a systemic disease that accelerates in the arteries of the whole body in the presence of diabetes. Some studies estimate that up to 35% of patients with atherosclerosis of lower extremity also suffer from significant stenosis of carotid arteries (> 50%), which is at high risk for cerebrovascular and cardiovascular events. Identification of such patients is very important, as up to 48% of strokes are caused by atherosclerosis of the carotid arteries. The ankle brachial index (ABI) method, which is based on the measurement of upper and lower limb pressures, is used to identify (screening) atherosclerosis of lower limbs (LEAD). The aim of this bachelor's thesis was to acquaint the reader with the issue of atherosclerosis of the carotid arteries in diabetics with LEAD, by characterizing individual diseases in the first (theoretical) part. This part was focused on: risk factors of these diseases and their clinical manifestations, origin and types of atherosclerotic plaque, anatomy and pathogenesis of carotid ATS, which plays an important role, and also especially the diagnosis of LEAD. For understanding of the issue, is main the second part (discussion), which summarizes as much scientific knowledge as possible about the relationship between atherosclerotic carotid disease and the proven presence of LEAD in diabetics. These scientific findings deal mainly with: the ABI method for the detection of LEAD, the newly investigated relationship between ABI and the detection of ATS carotid arteries and the characterization of ABI values. This work is written in the form of literary research. The information that was used comes from both professional Czech and professional foreign literature, which was mainly in electronic form. It was very difficult to evaluate the results of scientific studies because there were methodological differences in the studies. Nevertheless, diabetes has been found to be a major risk factor for atherosclerosis, and there is an association between LEAD and carotid ATS in diabetic patients. The results of the studies also showed that it is very important to check in diabetic patients with LEAD whether the patient also suffers from carotid ATS, or vice versa, patients who have had a stroke or have carotid artery stenosis (CAS) whether LEAD also suffers. The ABI value can play an important role in this context.
