The issue of clinically important enterobacteria producing extended - spectrum beta lactamases

Abstract:

Anotace Primárním cílem předložené práce bylo určit výskyt enterobakterií s produkcí širokospektrých beta-laktamáz v klinickém materiálu pacientů Fakultní nemocnice Olomouc pomocí fenotypových metod. Sekundárním cílem bylo stanovení četnosti širokospektrých beta-laktamáz typu ESBL, AmpC a karbapenemáz, u nejčastěji izolovaných enterobakterií. Výsledky předložené bakalářské práce potvrdily důležitost výše uvedených širokospektrých beta-laktamáz a dokumentují vysokou četnost klinicky významných enterobakterií s produkcí ESBL a AmpC enzymů. Nejvyšší výskyt byl prokázán u pacientů v intenzivní péči. Produkce ESBL enzymů byla prokázána častěji u kmenů Klebsiella pneumoniae, Escherichia coli a Proteus mirabilis. Produkce AmpC ezymů byla častěji prokazována u kmenů Enterobacter sp. Rezistence sledovaných enterobakterií k meropenemu, resp. karbapenemům, byla velmi nizká a tato antibiotika nadále představují vhodnou alternativu pro léčbu infekcí s etiologickou rolí enterobakterií produkujíích širokospektré beta-laktamázy. Přestože reálně byly izolovány enterobakterie rezistentní ke karbapenemům, tato rezistence nebyla podmíněna produkcí karbapenemáz.

Abstract:

The main goal of this thesis was to determine the occurence of enterobacteria producing extended – spectrum beta lactameses in the clinical material from patients of the University Hospital Olomouc, using fenotyping methods. A secondary goal was to determine the frequency of extended broad- spectrum beta lactameses of the types ESBL, AmpC and carbapenemeses at the most often isolated enterobacteria. The results of this thesis confirmed the importance of the above mentioned Extended -spectrum beta lactameses and illustrate the high frequency of clinically important enterobacteria producing ESBL and AmpC enzymes. The highest occurence was proved with the intensive care patients. The production of ESBL enzymes was proved most often at strains of Klebsiella pneumoniae, Escherichia coli a Proteus mirabilis. The production of AmpC enzymes was more often proved at strains of Enterobacter sp. Resistance of the monitored enterobacteria towards meropen or carbapenems was very low, and these antibiotics are still a suitable for the treatment of infections with etiologic role of enterobacteria producing extended -spectrum beta lactameses. Despite the fact that there were isolated enterobacteria resistant towards carbapenems, this resistance was not conditioned by the production of carbapenemases.
