Pediatric palliative care in a healthcare facility

Abstract:

Diplomová práce je zaměřena na dětskou paliativní a podpůrnou péči v nemocnici. Zaměřuje se na potřebu paliativního týmu u dětských pacientů s život limitujícím nebo ohrožujícím onemocněním, nepříznivou diagnózou nebo terminálním stavem onemocnění. V teoretické části se práce komplexně zabývá koncepcí dětské paliativní péče, jak s nimi komunikovat, jak sdělit závažnou diagnózu dítěti a rodině, v jakém prostředí a jak s nimi pracovat. Výstupem praktické části je potvrzení (na základě rozhovoru s rodiči pacientů s výše uvedenými onemocněními) potřeby mít dětský paliativní tým v nemocnici. Vytvoření dokumentační sady pro potřeby paliativního týmu, pomocí které můžeme identifikovat pacienta s potřebou paliativní a podpůrné péče a vytvořit plán této péče, představující metodického průvodce, je dalším výstupem práce. Zahrnuta je i potřeba pozůstalostní péče (rozlučkový list). Vytvoření návrhu této dokumentace je vítáno oslovenými odborníky a taková dokumentace bude dále rozvíjena ve strukturách paliativní a podpůrné péče (odborná společnost, jednotlivá centra).

Abstract:

The thesis is focused on pediatric palliative and supportive care in the hospital. It focuses on the need for a palliative team in pediatric patients with life-limiting or threatening disease, unfavorable diagnosis or terminal disease. In the theoretical part, the work comprehensively deals with the concept of pediatric palliative care, how to communicate with patients and families, how to communicate a serious diagnosis to the child and the family, in which environment and how to deal with them. The output of the practical part is the confirmation (based on the interview with parents of patients with the above diseases) of the need to have a pediatric palliative team in the hospital. Creating a documentation set for the needs of the palliative team, with which we can identify the patient with the need for palliative and supportive care and create a plan for this care, represents a methodological guide, is another practical outcome of the thesis. The need for survivors' care (farewell letter) is also included. The creation of a draft of this documentation is welcomed by the addressed experts and such documentation will be further developed in the structures of palliative and supportive care (professional society, individual centers).
