Burden on family members caring for the elderly with dementia

Abstract:

Cíl: Cílem práce bylo zjistit přítomnost a míru ošetřovatelské zátěže u rodinných příslušníků pečujících o seniory trpících demencí. Byly hodnoceny vztahy mezi mírou zátěže a sociodemografickými aspekty (věk a pohlaví pečovatele, délka poskytované péče rodinný vztah k pečované osobě). Byl hodnocen také vztah mezi zátěží a úrovní kognice a také soběstačností osob, o něž je pečováno. Metodika: Výzkumné šetření probíhalo formou kvantitativního výzkumu. Data byla sbírána pomocí standardizovaného dotazníku The Zarit Burden Interview - modifikovaný E. Topinkovou jako Test hodnocení pečovatelské zátěže pro rodinné pečující. Sběr dat probíhal v zařízení následné lůžkové péče a dále v domácnosti uživatelů terénní pečovatelské služby. Výzkumný soubor tvořilo celkem 104 rodinných pečujících, jenž pečovaly o osoby seniorského věku trpící demencí. K testování vztahu mezi zátěží pečovatele a úrovni soběstačnosti a kognitivního stavu nemocných, byl použit neparametrický Chí-kvadrát test, Test trendu a Fischerův exaktní test. Statistické testy byly hodnoceny na hladině významnosti 5 %. Zpracování bylo provedeno programem Stata v. 13. Výsledky byly zpracovány ve formě tabulek v MS Excelu. Výsledky: Výzkum analyzoval data 104 respondentů starších 18 let. V souboru pečujících osob převažovaly ženy 74 (71 %), nejčastěji ve věkové nad 65 let. Nejvíce pečujících poskytovalo péči mezi 1-3 roky. Výsledky prokázaly přítomnou zátěž různé intenzity u všech 104 (100 %) respondentů. Těžkou až extrémní zátěž označilo 15 (14 %) zkoumaných respondentů, střední zátěž označilo 86 (82,7 %) pečujících, tedy většina dotazovaných osob. Lehkou zátěž neoznačil nikdo z dotazovaných, pouze 3 (2,9 %) respondenti označili pociťovanou zátěž jako mírnou. Byl prokázán významný vztah mezi MMSE a zátěží (rs = 0,655; p < 0,001). Vztah mezi zátěží a věkem, pohlavím, délkou péče a mírou soběstačností nemocných nebyl prokázán. Závěr: Pečující osoby, jsou trvale vystaveny zátěži vyplývající z poskytované péče. Test hodnocení pečovatelské zátěže-The Zarit Burden Interviw, je nejpoužívanějším nástrojem k posouzení míry zátěže rodinných pečujících ve světě. Pomocí tohoto nástroje, můžeme mapovat oblasti dopadající zátěže, identifikovat náročné oblasti péče a zaměřit se na prevenci této zátěže a podporu pečujících rodin. Abstract:

Aim: The aim of this study was to determine the presence and level of nursing burden in family members caring for elderly people with dementia. The relationships between the level of burden and sociodemographic aspects (age and gender of the caregiver, length of care provided, family relationship to the cared-for person) were assessed. The relationship between burden and cognitive level as well as the self-sufficiency of the cared-for person was also assessed. Methodology: The research was conducted in the form of quantitative research. Data were collected using a standardized questionnaire, The Zarit Burden Interview - Modified E. Topinka as the Caregiver Burden Assessment Test for Family Caregivers. Data collection took place in the aftercare facility and in the home of the users of the outreach care service. The research population consisted of 104 family caregivers who cared for elderly persons suffering from dementia. To test the relationship between caregiver burden and the level of self-sufficiency and cognitive status of the patients, the non-parametric Chi-Square test, Trend Test and Fisher's exact test were used. Statistical tests were evaluated at the significance level of 5%. The processing was performed by Stata v. 13. The results were processed in the form of tables in MS Excel. Results: The research analysed data from 104 respondents over the age of 18. The majority of caregivers were female 74 (71%), most often over the age of 65. Most caregivers provided care between 1-3 years. Results showed the presence of burden of varying intensity in all 104 (100%) respondents. Severe to extreme burden was indicated by 15 (14%) of the respondents, while moderate burden was indicated by 86 (82.7%) caregivers, i.e. the majority of the respondents. Light burden was not indicated by any of the respondents, with only 3 (2.9%) respondents indicating perceived burden as moderate. There was a significant relationship between MMSE and burden (rs = 0.655; p < 0.001). There was no relationship between burden and patients' age, gender, length of caregiving, or level of self-sufficiency. Conclusion: Carers are permanently exposed to the burden of care. The Zarit Burden Interview is the most widely used tool to assess the burden of family caregiving in the world. Using this tool, we can map areas of impacted burden, identify challenging areas of caregiving, and focus on preventing this burden and supporting family caregivers.
