Adaptation of clinical practice guideline prevention of pressure ulcers

Abstract:

Cílem diplomové práce bylo metodou transkulturní adaptace zpracovat a adaptovat zahraniční klinické doporučené postupy prevence dekubitů a ověřit jejich použitelnost ve zdravotnických zařízeních v podmínkách České republiky. Pro adaptaci byly vybrány z veřejně dostupných databází a elektronických knihoven (např. G-I-N Guidelines International Network, Medline, ProQuest, Cochrane) kvalitativně nejvhodnější klinické doporučené postupy. Dalším krokem bylo KDP roztřídit a analyzovat. Metodikou nástroje ADAPTE (transkulturní adaptace) byla v první fázi hodnocena metodologická kvalita vybraných KDP generickým nástrojem AGREE II. Následně probíhala obsahová analýza, dalšími generickými nástroji ADAPTE. Nejlepší metodologickou i obsahovou kvalitu vykazoval KDP kanadské odborné společnosti registrovaných zdravotních sester (RNAO) Risk Assessment & Prevention of Pressure Ulcers. Konsensem odborníků pracovní skupiny, s respektem na extrahovaná data českého sociokulturního a organizačního kontextu, byl vybrán k překladu a implementaci. Přeložený adaptovaný klinický doporučený postup byl implementován do klinické praxe na vybraných pracovištích Městské nemocnice Ostrava. Po prvních třech měsících zatím nebyl zaznamenán významný rozdíl ve výskytu dekubitů před a po implementaci KDP. Celkové hodnocení implementace KDP bude provedeno na konci roku 2014.

Abstract:

The aim of this thesis was by the method of cross-cultural adaptation process to adapt international clinical guidelines to prevent of pressure ulcers and verify their applicability in health care facilities in the Czech Republic. For adaptation were selected qualitatively most suitable clinical guidelines from publicly available databases and electronic libraries (Guidelines International Network GIN, Medline, ProQuest, Cochrane). In the first stage by methodology ADAPTE (transcultural adaptation) was evaluated in the first phase of the methodological quality of selected guidelines by generic tool AGREE II. Subsequently conducted content analysis, other generic tools of ADAPTE. The best methodological quality and content showed KDP Canadian company specialized registered nurses (RNAO) Risk Assessment & Prevention of Pressure ulcers. Suitable guideline Canadian company specialized registered nurses (RNAO) was selected by consensus of the expert working group with respect to the extracted data of the Czech socio-cultural and organizational contexts. Translated adapted clinical guidelines has been implemented into clinical practice in selected departments of the City hospital Ostrava. After the first three months was not significant difference in the incidence of pressure ulcers before and after the implementation of KDP. Overall rating KDP implementation will be done by the end of 2014.
