Comparison of postoperative pain and patient self-sufficiency after regional and general anesthesia in Caesarean Section

Abstract:

Diplomová práce je zaměřena na vliv druhu podané anestezie na pooperační bolest a soběstačnost žen po císařském řezu. Práce je členěna na teoretickou a výzkumnou část. Teoretická část popisuje problematiku císařského řezu, jeho historii, vývoj, indikace i kontraindikace, způsob a metody provedení a možné komplikace. Dále se věnuje otázce porodnické anestezie, rozebírá možnosti, indikace, výhody, nevýhody a možné komplikace celkové, spinální a epidurální anestezie. V práci je popsána pooperační péče se zaměřením zejména na pooperační bolest, analgezii a soběstačnost žen po plánovaném i akutním císařském řezu. V rámci diplomové práce bylo provedeno výzkumné šetření, které probíhalo jako kvantitativní průzkum dotazníkovou formou. Výzkum byl proveden na Gynekologicko porodnické klinice Všeobecné fakultní nemocnice a I. lékařské fakulty Univerzity Karlovy v Praze v obdobích dubna až května 2018 a března až dubna 2019. Cílem výzkumné části diplomové práce bylo zjistit, zda typ anestezie ovlivňuje intenzitu a prožívání pooperační bolesti. Dalšími cíli bylo zjistit, jestli má druh anestezie vliv na výskyt perioperačních a pooperačních komplikací, pooperační návrat k soběstačnosti, zda jsou tyto faktory ovlivněny urgencí výkonu, managementem pooperační analgezie či dobou mobilizace.

Abstract:

This thesis is focused on the effect of the type of anesthesia administered on postoperative pain and self-sufficiency of women after caesarean section. The thesis is divided into a theoretical and a research part. The theoretical part describes the issue of caesarean section, its history, development, indications and contraindications, methods of implementation and possible complications. It also deals with the issue of obstetric anesthesia, analyzes the possibilities, indications, advantages, disadvantages and possible complications of general, spinal and epidural anesthesia. This thesis describes the postoperative care, which is focused on postoperative pain, analgesia and self-sufficiency of women after planned and acute caesarean section. The research was carried out as a quantitative survey in a form of a questionnaire. The research was carried out at the Department of Obstetrics and Gynecology of the General Faculty Hospital and the First Faculty of Medicine of the Charles University in Prague in the periods from March to April 2019. The main aim of the research part was to determine whether the type of anesthesia affects the intensity and experience of postoperative pain. Secondary aims were to find out whether the type of anesthesia affects the incidence of perioperative and postoperative complications, postoperative return to self-sufficiency, whether these factors are affected by the urgency of the procedure, the post-operative analgesia management, or the mobilization time.
