Mutational analysis of SF3B1 gene in myelodysplastic syndrome

Abstract:

Myelodysplastický syndrom (MDS) je charakterizován dysplazií kostní dřeně, která se projevuje cytopenií v periferní krvi. Onemocnění je značně heterogenní a dělí se na několik podskupin s rozdílnou prognózou a klinickým průběhem. Publikované studie potvrzují asociaci mutací v genu SF3B1 (splicing factor 3b subunit 1) s přítomností prsténčitých sideroblastů a současně s lepší prognózou onemocnění. Je proto snaha aberace detekovat jako součást prognostických vyšetření při diagnóze MDS. Přítomnost mutace v genu SF3B1 je od roku 2016 součástí WHO klasifikace. Cílem bakalářské práce bylo zavedení sekvenační analýzy exonu 14, 15 a 16, ve kterých se vyskytují nejčastější hot-spot mutace. Metodu jsme následně využili k vyšetření 58 MDS pacientů z IHOK FN Brno. U 17% pacientů jsme detekovali mutaci v genu SF3B1. Pozitivní prognostický dopad přítomnosti mutací na celkové přežívání jsme pozorovali, nicméně vzhledem k malému souboru pacientů bez statistické významnosti.

Abstract:

Myelodysplastic syndrome is characterized by dysplasia of bone marrow, which is manifested as a cytopenia in peripheral blood. This disease is very heterogeneous and is divided into several subgroups with different prognosis and clinical course. Published studies confirmed association of mutations in gene SF3B1 (splicing factor 3b subunit 1) with the presence of ring sideroblasts and correlation with better disease prognoses. There is an effort to detect aberration as a part of prognostic examination at the diagnosis of MDS. The presence of mutation in SF3B1 gene has been a part of the WHO classification since 2016. The aim of this bachelor thesis was the introduction of sequencing analysis of exons 14, 15 and 16, in which the most common hot-spot mutations occur. After that the method was used to examine 58 MDS patients from IHOK FN Brno. In 17% of patients, we detected a mutation in the SF3B1 gene. The positive prognostic impact of the presence of mutations on overall survival was observed, however due to a small group of patients without statistical significance.
