Analysis of a falls in a psychiatric hospital

Abstract:

Cíl: Cílem práce bylo retrospektivně zhodnotit výskyt pádů ve vybraném lůžkovém zařízení psychiatrické péče se zaměřením na faktory ovlivňující vznik pádů. U získaných dat bylo provedeno statistické vyhodnocení vybraných položek záznamového formuláře "Nežádoucí událost - pády pacienta" (demografická data, čas pádů, situace, za kterých k pádům docházelo a jejich důsledky, stanice přijímací, stanice následné péče). Metodika: Soubor tvořilo 2 662 záznamů o pádech z období 2013-2017. Získaná data byla statisticky zpracována pomocí statistického programu Stata verze 14. Pro statistické vyhodnocení hypotéz byl použit chí-kvadrát test pro dva a více výběrů. Statistické testy byly hodnoceny na hladině významnosti 5 %. Výsledky: Během zkoumaného období bylo hospitalizováno 37 714 pacientů. Zpracováním dat bylo zjištěno, že s vyšším věkem pacientů výskyt pádů u hospitalizovaných pacientů vzrůstá. Největší výskyt pádů byl zaznamenán ve věkové skupině 85 a více let 22,4 %. U žen bylo riziko pádů 1,7krát vyšší než u mužů. Nejčastější situace, za které docházelo k pádům byla nestabilita při chůzi na stanicích přijímacích (27,7 %) a na následné péči (24,3 %). Byl zjištěn statisticky významný rozdíl v rozložení pádů dle časového rozmezí mezi typem stanic. Na příjímacích stanicích je vyšší frekvence pádů během noci (29 %), na následné péči pak vyšší počet zaznamenáváme přes den (44 %). Nebyl potvrzen statisticky významný rozdíl dle věku a podílu pádů se zraněním z celkového počtu 2 662 pádů. Výskyt závažných zranění byl nejvyšší u pacientů v kategorii 75-84 let (20 %). Závěr: Z analýzy dat vyplynulo, že pády jsou závažným problémem v psychiatrické nemocnici. Vzhledem k aktuálnosti tématu a četnosti pádů pacientů v psychiatrickém prostředí nemocnice je nutné věnovat pozornost problematice pádů hospitalizovaných pacientů a inovovat a zefektivňovat postupy, které budou pády pacientů nadále eliminovat.

Abstract:

Objective: The aim of my thesis was to retrospectively evaluate the incidence of falls in a selected inpatient psychiatric hospital, with a special focus on factors affecting the occurence of falls. Selected items of the data collected in the form of completed patient record forms "Adverse event - patient's falls" were evaluated statistically (i.e. demographic data, time of falls, situations during which the falls occured, their consequences, Admissions department, follow-up care department). Methods: The total of 2 662 records of falls from in the period of 2013-2017 were analysed. The collected data were statistically processed by means of statistical software Stata, version 14. The hypotheses were statistically evaluated by chi-square test for two or more samples. The statistical tests were evaluated at the level of significance of 5 %. Results: During the study period the total of 37 714 patients were hospitalized. The data analysis demonstrated that the incidence of falls increases with a higher age of hospitalized patients. The most frequent incidence of falls, i.e. 22.4 %, was recorded in the age group of 85 years and older patients. The risk of fall in female patients was 1.7 times higher than in male patients. The most frequent situations of falls was instability when walking in the Admissions departments (27.7 %), and in the follow-up care departments (24.3 %). A statistically significant difference was found in the distribution of falls by time frames between the types of departments. A higher incidence of falls was reported in the Admissions department during the nights (29 %), whereas in the follow-up care department a higher incidence of falls was reported during the days (44 %). No statistically significant differences were found between the age groups and between the proportions of falls with injuries out of the total number of 2 662 falls. The incidence of severe injuries was the highest in the age group of 75-84 years (20 %). Conclusion: The data analysis proved that falls are a serious problem in psychiatric hospitals. With regard to the topicality of this problem and the frequency of patients' falls in the psychiatric hospital setting it is necessary to focus on the problem of falls in hospitalized patients, innovate procedures which will gradually reduce the number of patients' falls and make these procedures more effective.
