Abstract:

ABSTRAKT Úvod: Ošetřovatelská diagnostika má zásadní význam pro sestry ve všech oblastech zdravotní péče, protože přesnost ošetřovatelských diagnóz ovlivňuje efektivitu ošetřovatelské péče. Cíl: Cílem disertační práce bylo zjistit nejfrekventovanější ošetřovatelské diagnózy ve FN Ostrava, identifikovat nejčastější problémy při formulování diagnostického závěru včetně identifikace diagnostických prvků ze systému NANDA International. Zjistit podloženost diagnostického závěru relevantními údaji, hodnotícími nástroji. Metodika: Na sběr empirických údajů byla použitá metoda analýzy zdravotnické dokumentace. Frekvence ošetřovatelských diagnóz z jednotlivých domén klasifikačního systému NANDA International byla zjištěna retrospektivní analýzou zdravotnické dokumentace. Pro měření diagnostické kvality byl použit modifikovaný hodnotící nástroj Quality of Nursing Diagnosis (QOD). Výsledky: Celkem bylo nalezeno 1696 diagnostických závěrů, bylo použito 50 opakujících se ošetřovatelských diagnóz; 29 aktuálních, 10 potenciálních a 11 diagnóz nebylo formulováno standardizovanou ošetřovatelskou terminologií. V 86,4 % byly identifikovány diagnózy podle NANDA-I. Byly použity diagnózy z šesti domén (Výživa, Vylučování/výměna, Aktivita/odpočinek, Vnímání/poznávání, Bezpečnost/ochrana, Komfort). Použitím nástroje QOD bylo identifikováno 1489 chyb, převažovaly chyby vztahující se k relevantnosti nad chybami v oblasti struktury. Značná část chyb byla identifikována v oblasti formulace ošetřovatelské diagnózy a v komponentu etiologie. V oblasti podloženosti diagnostického závěru relevantními údaji či hodnotícími nástroji, nebylo téměř 27 % ošetřovatelských diagnóz podloženo relevantními údaji, především potenciální diagnózy, které jsou automaticky stanovovány v souvislosti s určitou medicínskou diagnózou, operačními či jinými invazivními výkony nebo potenciálními komplikacemi léčby. Závěr: Ve sledovaném souboru převládaly ošetřovatelské diagnózy taxonomie NANDA International, v několika případech byly identifikovány nestandardizované ošetřovatelské pojmy, z čehož dva se často opakovaly. V souboru absentovaly diagnózy z domény Podpora zdraví, Sebepercepce, Zvládání zátěže a Životní principy. Při použití hodnotícího nástroje QOD převládaly chyby vztahující se k relevantnosti diagnostických prvků nad jejich strukturou. Největší problémy se vyskytovaly v oblasti objektivizace ošetřovatelských diagnóz hodnotícími škálami, ve formulaci diagnostických pojmů a v identifikaci souvisejících faktorů.

Abstract:

ABSTRACT Introduction: Nursing diagnosis is essential for nurses in all areas of health care, because the accuracy of nursing diagnoses affecting the effectiveness of nursing care. Aim: The aim of the dissertation thesis was to determine the most frequent nursing diagnoses at the University Hospital Ostrava, and to identify the most frequently observed problems with formulation of the diagnostic conclusion, including identification of diagnostic elements from the NANDA International system. Furthermore, to determine the substantiation of the diagnostic conclusion with relevant data and evaluation tools. Methods: The method of medical records analysis was used to collect the empirical data. The frequency of nursing diagnoses from individual domains of the NANDA International classification system was determined with retrospective analysis of medical records. The Quality of Nursing Diagnosis (QOD) modified evaluation tool was used to measure the diagnostic quality. Results: A total of 1696 diagnostic conclusions were identified, a total of 50 repeating nursing diagnoses were used; 29 actual, 10 potential, and 11 diagnoses were not formulated with the standard nursing terminology. Diagnoses according to the NANDA-I system were identified in 86.4 per cent of cases. Diagnoses from six domains were used (Nutrition, Elimination/Exchange, Activity/Rest, Perception/Cognition, Safety/Protection, Comfort). By using the QOD tool, a total of 1489 errors were identified; errors pertaining to relevancy prevailed over the errors in structure. Significant proportion of the errors was identified in the area of formulation of the nursing diagnosis and in the component of aetiology. In the area of substantiation of the diagnostic conclusion with relevant data or evaluation tools, almost 27 per cent of the nursing diagnoses were not substantiated with relevant data, namely potential diagnoses, which are automatically determined in relation to a certain medical diagnosis, surgical or other invasive procedures, or potential complications of the treatment. Conclusion: Nursing diagnoses of the NANDA International taxonomy prevailed in the observed file, non-standardized nursing terms were identified in several cases, two of these repeated frequently. Diagnoses from the domains of Health Promotion, Self-Perception, Coping/Stress Tolerance and Life Principles were missing in the analysed group. When using the QOD tool, errors related to the relevancy of the diagnostic elements prevailed over errors pertaining to their structure. The greatest problems were observed in the area of objectification of nursing diagnoses with assessment scales, in formulation of diagnostic terms and identification of related factors.
