Nutrition of seniors and prevention of selected diseases

Abstract:

Po celém světe dochází k nárůstu podílu starší populace. Zásluhou pokroků vědy, techniky a nových poznatků v oblasti medicíny se průměrná délka lidského života, zejména v západních zemích, prodlužuje. Lidské tělo je během procesu stárnutí vystaveno změnám, které se negativně odráží na mnoha funkcích tělesných soustav a zvyšují náchylnost staršího jedince k rozvoji nemocí. Úbytek kostní hmoty, pokles svalové síly, kognitivní pokles a narušení funkce gastrointestinální soustavy jsou jen příklady změn, které mohou být procesem stárnutí vyvolané. Pochopení procesu stárnutí a mechanismů, jež změny vyvolávají, přispívá k porozumění nutričních potřeb starší populace. Výživová doporučení určená starší populaci mohou být efektivní strategií pro snížení dopadu či dokonce zabránění negativních změn. Nesprávné stravovací návyky u starší populace končí rozvojem obezity nebo podvýživy a s nimi přidruženými komplikacemi. U starší populace je vysoká prevalence chronických onemocnění jako např. osteoporóza, diabetes mellitus, kardiovaskulární a neurodegenerativní onemocnění. Pacienti s těmito diagnostikovanými chorobami představují zátěž pro zdravotní sektor i ekonomiku. Z takového důvodu je v zájmu celého světa stanovit nejefektivnější preventivní doporučení. Celá práce směřovala ke splnění cíle, kterým bylo z poznatků uveřejněných v české i zahraniční literatuře sepsat doporučení týkající se výživy seniorů a preventivních opatření vybraných onemocnění. Bylo zjištěno, že výživa má zásadní vliv pro udržení optimálního zdravotního stavu a dobré kvality života staršího jedince. Dále bylo zjištěno, že nutriční potřeby se vlivem působení procesu stárnutí mění a jejich naplnění může výrazně přispět k prevenci rozvoje řady onemocnění. U starších osob jsou zvýšené nároky na příjem zejména u bílkovin, vitaminu D, B12, vápníku a zinku. Naopak příjem fosforu je u většiny populace hodnocen jako nadbytečný. Neznalost a ignorování výživových doporučení pro starší populaci má za následek rozvoj malnutrice, jež zahrnuje nadváhu, obezitu, podvýživu, anebo deficit mikronutrientů. Takové stavy jsou často doprovázeny řadou komplikací a ztěžují běžný život jedince. Co se týče prevence vybraných onemocnění, zde bylo zjištěno, že příjem určitých živin ve stravě a celkové složení stravy se jeví jako efektivní preventivní strategie. Součástí preventivních opatření ovšem není pouze změna stravovacích návyků, ačkoliv je to velmi významný aspekt prevence, ale úpravy by se měly týkat celkového životního stylu. S prevencí je možné začít v jakékoli etapě lidského života, avšak předpokládá se, že největších účinků lze dosáhnout při přijetí preventivních opatření v mladším věku jedince.

Abstract:

The proportion of the elderly across the world is increasing. Thanks to advances in science, technology and new medical knowledge, life expectancy is way higher than it was few years ago. Especially in Western countries. During the aging process, the human body is subject to changes that adversely affect many functions of the body systems and increase the susceptibility of older individuals to the development of diseases. Loss of bone mass, decline in muscle strenght, cognitive decline and impaired gastrointestial function are just a few examples of the changes that can be inducted by the aging process. Understanding the aging process and and the mechanisms that trigger the changes contributes to understanding the nutritional needs of the elderly population. Nutritional recommendations for the elderly population can be an effective strategy to reduce the impact or even prevent negative changes completely. Poor dietary habits in the elderly population result in the development of obesity or malnutrition and associated complications. The elderly population has high prevalence of chronic diseases such as osteoporosis, diabetes mellitus, cardiovascular and neurodegenerative diseases. Patients with these diagnosted diseases represent a burden on the health sector and the economy. It is therefore in an interest of the world to determine the most effective recommendations to prevent all this from hapenning. The whole work was aimed at fulfilling the goal, which was to use the knowledge published in the Czech and foreign literature to write recommendations concerning nutrition of the elderly and preventive measures for selected diseases. It was found that nutrition is essential for maintaining optimal health and a good quality of life in the elderly. It was also found that nutritional needs change due to the effects of the aging process and meeting them can contribute significantly to the prevention of the development of a number of diseases. In particular, the intake requirements of protein, vitamin D, B12, calcium and zinc are increased in the elderly. In contrast, phosphorus intake is assessed as excessive in most of the population. Ignorance or disregard of nutritional recommendations for the elderly population results in the development of malnutrition, which includes overweight and obesity, malnutrition, and/or micronutrient deficiencies. Such conditions are often accompanied by number of complications which are making it difficult for an individual to live a normal life. With regard to the prevention of selected diseases, the intake of certain nutrients in the diet and the overall composition of the diet have been found to be effective preventive strategies. However, preventive measures do not only include changes in dietary habits, although this is a very important aspect of prevention, but should also include adjustments to the overall lifestyle. Prevention can start at any stage of a person's life, but it is believed that the greatest effects can be achieved when preventive measures are taken at a younger age.
