Psychic and physical strain of nurses in the University Hospital in Ostrava

Abstract:

Cíl: Cílem diplomové práce bylo zjistit psychickou a fyzickou zátěž sester pracujících na lůžkových odděleních ve Fakultní nemocnici Ostrava. Posoudit vztah mezi zátěží a demografickými faktory (věk, délka praxe a dosažené vzdělání), a zjistit rozdíly v zátěži mezi sestrami akutních a standardních pracovišť a oddělení interních a chirurgických oborů. Metodika: Soubor tvořilo 400 sester pracujících na lůžkových odděleních a klinikách Fakultní nemocnice Ostrava. Pro sběr dat byly použity dva standardizované dotazníky - Meisterův dotazník pro hodnocení psychické zátěže a Baeckeho dotazník habituální pohybové aktivity. Pro základní popis souboru byla použita popisná statistika (medián, aritmetický průměr, směrodatná odchylka, frekvenční tabulky s absolutními a relativními počty). Pro testování hypotéz byl použit chí-kvadrát test, Fisherův exaktní test. Pro testování vztahu mezi psychickou a fyzickou zátěží byl použit Spearmanův korelační koeficient. Statistické testy byly hodnoceny na hladině významnosti & = 0,05. Pro zpracování dat byl použit program STATA verze 13. Výsledky: Na základě analýzy výsledků byl prokázán vztah psychické i fyzické zátěže s věkem a délkou praxe. Vliv na zátěž sester má dlouhodobá únosnost, časová tíseň a vysoká odpovědnost. Sestry standardních oddělení při své práci pociťuji únavu. Rozdíl v hodnocení zátěže byl prokázán mezi sestrami standardních a akutních pracovišť ve faktoru přetížení. U celého souboru nedošlo k překročení stupně přetížení 1. Závěr: Organizace by se měla podílet na vytváření takových pracovních podmínek, které by nepřispívaly k nárůstu psychické a fyzické zátěže sester. Důraz by měl být kladen na osvětu v duševní hygieně. Sestry by měly mít možnost využít péči krizového interventa.

Abstract:

Aim: The aim of this dissertation is to determine the psychological and physical stress in nurses working on wards at the University Hospital of Ostrava, to assess the relationship between stress and demographic factors (age, length of service and education attained), and to identify differences in stress between nurses at standard and emergency workplaces as well as between medical and surgical disciplines. Methodics: A sample of 400 nurses working on wards and in clinics at the University Hospital Ostrava was created. For data collection, two standardised questionnaires were used ? the Meister questionnaire for assessing psychological stress and the Baecke habitual physical activity questionnaire, and for a basic description of the sample, descriptive statistics were used (median, mean average, standard deviation, and absolute and relative frequency tables). To test the hypothesis, a chi-squared test and Fisher?s exact test were used whilst to test the relationship between psychological and physical stress, Spearman?s rank correlation coefficient was used. Statistical tests were assessed at a significance level of 0.05. STATA version 13 was used for data processing. Results: Based on an analysis of the results, a relationship between both psychological and physical stress and age and length of service has been proven. Long-term tolerability, time constraints and high responsibility have an impact on stress among nurses. Nurses working in standard departments feel fatigue during their work. During the evaluation of stress, a difference in the work overload factor was found between nurses who work at standard and emergency workplaces. Nowhere in the entire sample was degree of work overload 1 exceeded. Conclusion: The organisation should help to create working conditions which will not contribute to the growth of psychological and physical stress among nurses. Emphasis should be placed on raising awareness of mental hygiene and nurses should have access to the care of an interventionist.
