Artificial Neural Network Prediction for Cancer Patients Survival

Abstract:

Tato diplomová práce se věnuje individuální predikci jednoletého, tříletého a pětiletého přežití pacientů s kolorektálním karcinomem. Individuální predikce je zde uchopena jako klasifikační problém, který je řešen pomocí umělých neuronových sítí. Jako srovnávací metoda zde byla použita logistické regrese. V první části se práce věnuje problematice kolorektálního karcinomu. Je zde zmíněna epidemiologie této nemoci, etiologie, symptomy a stručně i diagnostika a léčba. Tyto informace jsou spolu s literárními zdroji posléze použity pro výběr prediktivních proměnných. Po seznámení se s pozadím klasifikačního problému následuje popis použitých metod, tedy krátký popis binární logistické regrese a popis hlavní používané metody – umělých neuronových sítí. Ten je zaměřen především na dopředné MLP sítě, které jsou používány pro výpočty v této práci, jejich stavbu, učení a implementaci v softwaru SPSS. Doplněna je i kapitola o vyhodnocování výsledků individuálního přežití. Data použitá v této práci pocházejí z Národního onkologického registru České republiky. Konkrétně se jedná o onkologické pacienty s diagnózou C18 (rakovina tlustého střeva). Další kapitoly práce se tak věnují krátkému seznámení čtenáře s Národním onkologickým registrem a povahou dat. Následuje také popis výběru proměnných z těchto dat používaných při samotné klasifikaci. Poslední část práce se věnuje výsledkům predikce přežití u individuálních pacientů. Úspěšnost je interpretována zvlášť u jednoletého, tříletého a pětiletého přežití na obou klasifikačních metodách. Následně jsou výsledky shrnuty a je porovnávána úspěšnost umělých neuronových sítí a logistické regrese, vliv vyřazení některých proměnných, vliv vyřazení pacientů zemřelých na jinou diagnózu nebo pacientů s nevyplněnými klasifikačními charakteristikami. Výsledky jsou také srovnány s obdobným výzkumem provedeným v zahraničí.

Abstract:

This thesis is devoted to the individual prediction one-year, three-year and five-year survival of patients with colorectal cancer. Individual prediction is in this thesis perceived as a classification problem, which is solved by using the method of artificial neural networks. As a comparative method there was used the logistic regression. The first part of this thesis deals with the subject of colorectal cancer. The epidemiology of the disease is mentioned as well as its etiology, symptoms, and briefly even diagnosis and treatment. This information together with literature sources are subsequently used for the selection of predictive variables. The introduction of the background of the classification problem is followed by a description of the methods used. There is a short introduction to binary logistic regression and also a description of the main methods used - artificial neural networks. The description is focused on the feedforward MLP networks which are used for the calculations in this thesis, their construction, learning and implementing in the SPSS software. A chapter on evaluating the results of individual survival is also added. The data used in this study were provided by the National Oncological Register of the Czech Republic; specifically cancer patients diagnosed with C18 (colon cancer). Another chapter is devoted to a brief introduction of the National Oncological Register and the nature of the data. Then there is a description of the selection of variables from the data used in the actual classification. The last part deals with the prediction of survival for individual patients. A success rate is interpreted separately for a one-year, three-year and five-year survival with both classification methods. Subsequently, the results are summarized and there are compared the success rate of artificial neural networks and logistic regression, the effect of the exclusion of certain variables and the impact of exclusion of patients deceased as a result of different diagnosis or patients with incomplete classification characteristics. The results are compared with similar research conducted abroad.
