Keratoconus patients after CXL treatment in the view of confocal microscopy

Abstract:

Abstrakt CÍLE PRÁCE: Zjištění hustoty keratocytů v rohovce s keratokonem a porovnání se zdravou populací. Sledování změn v počtech keratocytů a endotelových buněk před CXL a v následujícím dvouletém sledovacím období po zákroku. Ověření hloubky, do které působí CXL smrt buněk a zjištění průběhu repopulace. METODY: Pomocí konfokálního mikroskopu Confoscan 4 byly sledovány změny v počtech keratocytů v celé šíři rohovkového stromatu před CXL a dále pak 1,5 měsíce, 3, 6, 12 a 24 měsíců po CXL. Soubor obsahoval 59 očí (28 OS, 31 OD) 46 pacientů s keratokonem; kontrolní vzorek 26 očí (12 OS, 14 OD) 14 osob. Medián věku souboru byl 28 let. VÝSLEDKY: Počet endotelových buněk se u keratokonických a zdravých rohovek neliší (P = 0,3533). Hustota keratocytů je nejvyšší v anteriorní části stromatu (139,2 ± 4,319) a posteriorním směrem hustota klesá (112,6 ± 3,843). Klesající trend je vidět pouze u zdravých rohovek, v keratokonických rohovkách je hustota keratocytů v celé šíři stromatu stejná. U keratokonu totiž prodělávají keratocyty apoptózu i bez jakéhokoli epitelového poranění. Rozdíl průměrů počtů keratocytů v jednotlivých hloubkách při srovnání keratokonických a zdravých rohovek je 31,11 (P < 0,0001). Vlivem CXL nedochází k poškození endotelových buněk, neboť energie záření klesá směrem k endotelu až pod hraniční mez. Smrt keratocytů po CXL je pozorovatelná do hloubky 270–390 μm. Pod touto hladinou jsou populace keratocytů nenarušeny. Repopulace začíná již 1,5 měsíce po CXL. Počet keratocytů se zvyšuje až do šestého měsíce po CXL. Od této doby průběh repopulace stagnuje, neboť se počet buněk nezvyšuje. Předoperační hustotu tak rohovka nenabývá ani 24 měsíců po CXL. V hloubce stromatu těsně pod epitelem se počet buněk statisticky vysoce významně snížil z 98,66 ± 5,016 před operací na 40,00 ± 8,008 buněk 24 měsíců po CXL. ZÁVĚR: Excesivní apoptóza keratocytů v keratokonické rohovce způsobuje snížení jejich počtu. Apoptóza je nejvýraznější v anteriorním stromatu. CXL způsobuje smrt keratocytů až do hloubky 270–390 μm, buňky pod touto hladinou nejsou zářením nijak poškozeny. Repopulace začíná 1,5 měsíce po CXL a je ukončena již šest měsíců po CXL. Předoperační kvantita keratocytů se však neobjevuje ani dva roky po operaci. Abstract:

Abstract PURPOSE: Analysis of keratocyte density in keratoconus cornea and comparison with control sample of healthy population. Detection of changes in the keratocyte and endothelial cell density before CXL and in the following two years period after treatment. Verifying the corneal depth of keratocyte apoptosis after CXL and analysis of following keratocyte repopulation. METHODS: In vivo confocal microscopy has been used for imaging the whole corneal stroma thickness. We have followed changes in keratocyte density before CXL and 1,5 month, 3, 6, 12, 24 months after treatment. 59 eyes (28 OS, 31 OD) of 46 keratoconus patients were observed; 26 eyes (12 OS, 14 OD) of 14 people were assessed. Median of age in cohort was 28 years. RESULTS: Difference between endothelial cells count of keratoconus and normal corneas is not statistically significant (P = 0,3533). Density of keratocytes is highest at anterior stroma with 139,2 ± 4,319 cells and declines to posterior stroma up to 112,6 ± 3,843 cells. Downward trend was present only at normal corneas. In keratoconus corneas was the density of keratocytes same in whole thickness of the cornea. Keratocytes in keratoconus cornea undergo more often apoptosis even in case of no epithelial injury. Mean difference of keratocyte density in comparison of keratoconus and normal cornea was in various depths 31,11 (P < 0,0001). Endothelial cells are not injured by CXL while the intensity of radiation decreases to epithelium under the danger level. Keratocyte death is detectable to the depth 270–390 μm. Under this level keratocytes are intact. Repopulation begins 1,5 months after CXL treatment. Keratocyte density rises up to six months after CXL. Period of stagnation comes after this rise. Preoperative density of keratocytes is not present even two years after CXL treatment. Keratocyte density in the stromal depth just under epithelium got statistically significant declined from preoperative 98,66 ± 5,016 cells to 40,00 ± 8,008 cells 24 months after CXL treatment. CONCLUSION: Excessive keratocyte apoptosis causes decrease of keratocyte density in all stroma thickness in keratoconus cornea. This trend is most evident in anterior stroma. CXL induces keratocyte death to 270–390 µm of the corneal depth. Cells under this level are not harmed. Repopulation begins 1,5 months after CXL and follows up to 6 months after treatment. Preoperative keratocyte density is not present even two years after CXL treatment.
