The availabity of medical care in Czech Republic- The legislation versus reality and perspective versus the actual requirements

Abstract:

Úvod: Diplomová práce se zabývá dostupností zdravotních služeb v ČR, která je garantovaná právní úpravou. Spokojenost obyvatel, resp. pacientů se systémem zdravotní péče je cílem zdravotní politiky vyspělých zemí, a má mít zájem o zdravotní stav obyvatel. Najít poskytovatele zdravotních služeb v rozumné vzdálenosti a současně čekání na stanovení správné diagnózy - vyšetření by nemělo být příliš složité nebo nepřiměřeně dlouhé. Cíl práce: Cílem práce bylo zjištění znalosti obyvatel ČR v otázce místní a časové dostupnosti zdravotní péče, příčin nedostupnosti zdravotní péče, osobní zkušenosti s následným zajištěním požadované péče a zjištění, jaký obor zdravotní péče je vnímán jako nejméně dostupný. Metodika: Diplomová práce obsahuje teoretickou a výzkumnou část. V teoretické části jsou zahrnuty informace o systému a financování zdravotních služeb, zařazování poskytovatelů zdravotních služeb do sítě, dostupnost zdravotní péče, její právní rámec a sledování dostupnosti zdravotními pojišťovnami. Výzkumná část se věnuje znalosti povědomí obyvatel týkající se dostupnosti zdravotních služeb, jejich zkušeností. Zjištění bylo realizované pomocí kvantitativního dotazníkového šetření a následná data byla dále pomocí popisné statistiky vyhodnocena písemně a graficky. Výsledky: Neznalost legislativy, která se týká dostupnosti zdravotních služeb uvedlo 55,40 % respondentů. Přesto v otázce zajištění zdravotní péče by se 61,00 % respondentů obrátilo na správnou instituci, a to ZP. Nedostupnost pouze v některých oborech/službách vnímá 68,54 % respondentů. Oblast specializované ambulantní péče byla respondenty vyhodnocena v 36,20 % jako oblastí s největším problémem v zajištění dostupnosti péče. Důvod nedostupnosti spatřují respondenti v 61,03 % v nedostatku lékařů/zdravotnických pracovníků. Současně je zajímavé, že obdobné procento respondentů s bydlištěm ve městě 50,91 % či na venkově 50,63 % se setkalo s nedostupností zdravotní péče. Jako obor s největší nedostupností u respondentů, kteří se setkali s nedostupností byl respondenty označen v 41,90% obor stomatologie. Kritérium "dobré reference o lékaři" při výběru lékaře uvedlo 98,12 % respondentů. Závěr a doporučení: Dostupnost, resp. nedostupnost zdravotní péče v ČR není jen o místní a časové dostupnosti, ale je to komplexní nahlížení na zdravotní služby. Je nutné nastavit parametry vzájemné spolupráce již od prvních kroků na lékařských fakultách až po podávání informací o volné kapacitě pro poskytnutí péče/služby a rovněž také v lepší koordinaci spolupráce všech stakeholderů v systému veřejného zdravotního pojištění, včetně zapojení aktérů z oblasti systému sociálních služeb, školství, dopravní obslužnosti a místního rozvoje.

Abstract:

Introduction: The diploma thesis deals with the availability of health services in the Czech Republic, which is guaranteed by legal regulation. Citizens' contentment, or of patients with the health care system is the goal of the health policy of developed countries, and it should be interested in the health status of the population. Finding a health care provider within a reasonable distance while waiting for the correct diagnosis - the physical examination should not be too complicated or unreasonably long. Aim of the thesis: The goal of the thesis was to find out about the local and temporal availability of health care, the causes of health care, personal experience with the subsequent provision of the required care and to find out which field of health care is perceived as the least accessible. Methodology: The diploma thesis contains a theoretical and research part. The theoretical part includes information on the system and financing of health services, inclusion of health service providers in the network, availability of health care, its legal framework and monitoring of availability by health insurance companies. The research part is devoted to knowledge of citizens' awareness regarding the availability of health services, their experiences. The findings were realized using a quantitative questionnaire survey, and the subsequent data were further evaluated in writing and graphically using descriptive statistics. Results: 55,40 % of respondents reported ignorance of the legislation regarding the availability of health services. Nevertheless, when it comes to ensuring health care, 61,00 % of respondents would turn to the correct institution, namely health insurance. 68,54 % of respondents perceive unavailability only in some fields/services. The area of specialized ambulatory care was evaluated by 36,20 % of the respondents as the area with the greatest problem in ensuring the availability of care. 61,03 % of the respondents see the reason for the unavailability as a lack of doctors/medical workers. At the same time, it is interesting that a similar percentage of respondents living in the city 50,91 % or in the countryside 50, 63 % encountered the unavailability of health care. 41,90 % of the respondents identified the field of dentistry as the field with the greatest unavailability among respondents who encountered unavailability. 98,12 % of respondents mentioned the criterion of "good references about the doctor" when choosing a doctor. Conclusion and recommendations: Availability, or the lack of access to health care in the Czech Republic is not only about local and temporal availability, but a comprehensive view of health services. It is necessary to set the parameters of mutual cooperation from the first steps at the faculties of medicine to the provision of information on free capacity for the provision of care/service, as well as in better coordination of the cooperation of all stakeholders in the public health insurance system, including the involvement of actors from the field of social services, education , transport services and local development.
