Ethics of Resuscitation at Different stages of Life

Abstract:

Cíl: Cílem diplomové práce bylo zmapovat problematiku etického rozhodování zahájení kardiopulmonální resuscitace v různých fázích života. Hlavním cílem bylo zjistit, v jakém pořadí by respondenti zahájili neodkladnou resuscitaci a výsledky porovnat s již existujícími zahraničními studiemi. Dílčími cíli bylo zjistit, do jaké míry ovlivňují rozhodovaní o pořadí zahájení resuscitace faktory jako je víra, věk, vlastní děti či profese respondentů. Dalším dílčím cílem bylo zjistit, zda existuje rozdílný postoj respondentů o zahájení neodkladné resuscitace vzhledem k vlastnímu či neznámému dítěti. Metodika: K získání potřebných informací bylo zvoleno kvantitativní výzkumné šetření pomocí standardizovaného dotazníku autorek A. Janvier a K. J. Barrington, ve kterém bylo predikováno osm nekompetentních pacientů různého věku s potencionálními neurologickými následky, morbiditou a mortalitou, kteří vyžadovali zahájení kardiopulmonální resuscitace. Elektronickým anonymním dotazníkem bylo osloveno 200 respondentů. Výzkumné šetření probíhalo od listopadu 2017 do května 2018. Následně byla získaná data statisticky zpracována. Výsledky: Výzkumným šetřením byly stanovené cíle diplomové práce splněny. Ze získaných údajů je patrné, že v rozhodování zahájení resuscitace jsou znevýhodňováni pacienti na začátku života, tedy novorozenci. Také pacienti na sklonku života jsou v resuscitaci opomíjeni, protože často je jejich polymorbidita vnímána negativně. Dále bylo zjištěno, že u všech pacientů nezanedbatelná část respondentů považuje za vhodné zahájení léčby přesto, že se domnívá, že to není v souladu s nejlepšími zájmy pacienta. Dalším překvapivým zjištěním byl fakt, že na základě přání rodičů či rodiny nemocného by respondenti téměř u všech pacientů změnili terapeutický přístup. Nejvíce pak u pacientů novorozeneckého věku. Ve změně postoje zvažování zahájení resuscitace u vlastního a neznámého dítěte byl ve studii zjištěn významný rozdíl u případů obou novorozenců, kdy by respondenti poskytli péči neznámému dítěti a u vlastního dítěte by si zahájení léčby nepřáli. Výzkumným šetřením se nepodařilo prokázat, že by na rozhodování respondentů měly podstatný vliv jejich víra, věk, pohlaví, profese či fakt, zda mají či nemají vlastní děti Závěr: Při srovnání výsledků studie s publikovanými údaji jsou zachyceny podobné trendy, rozdíly však nejsou tak výrazné. Může to být způsobeno odlišnostmi ve zdravotnickém i právním systému (Kanada, Česká republika) i odlišnými názory veřejností na jednotlivé zásadní otázky týkající se intenzivní péče a resuscitace, tabuizace některých témat (úmrtí) atd.

Abstract:

Objective: The goal of this thesis was to map the problems of ethical decision-making associated with the initiation of cardiopulmonary resuscitation of patients in different life stages. The main objective was to find out how the respondents would rank patients in order of priority for immediate cardiopulmonary resuscitation and compare the findings with existing international studies. A further objective was to determine to what extent factors such as religion, age, profession or having children affect the respondents' decision-making. Another objective was to find out if there was a difference in the respondents' attitude towards the initiation of immediate cardiopulmonary resuscitation of their own vs. somebody else's child. Methodology: To gain the required information, we chose to conduct a quantitative research using the standardized questionnaire by Janvier and Barrington, focused on eight incompetent patients of different ages with potential neurosurgical implications, morbidity and mortality requiring the initiation of cardiopulmonary resuscitation. The anonymous electronic survey was distributed to a total of 200 respondents. The research was conducted from November 2017 to May 2018. The acquired data was statistically processed. Results: The results obtained by applying the methodology met the objectives of the thesis. The acquired data shows that patients at the beginning of life, i.e. newborns, are disadvantaged. Patients at the end of life also tend to be overlooked when considering resuscitation because their multiple morbidity is often perceived negatively. It was also found that in all patients, a significant part of the respondents consider it appropriate to initiate treatment even though they believe it is not in the best interest of the patient. Another surprising finding was that in nearly all cases, the respondents were ready to change the therapeutic attitude according to the wish of the patient's parents or family. The parents' wish was given the most weight when the patient in question was a newborn. Concerning a potential change of attitude towards resuscitation of their own vs. somebody else's child, a significant difference was observed in both newborn patients, with respondents opting against treatment in the case of their own child but for treatment in the other case. Factors such as religion, age, gender, profession or having children did not prove to play any role in the respondents' decision-making. Conclusion: Comparing the obtained results with other published data, similar trends can be observed but the differences are not as prominent. This could be explained by differences between the Canadian and Czech healthcare and legal systems, as well as different public opinions on fundamental questions related to the topics of intensive care and resuscitation, taboo topics such as death and other.
