Psychological Support for Mothers of Premature Infants in the Neonatal Intensive Care Unit

Abstract:

Diplomová práca sa zaoberá prežívaním matiek v súvislosti s predčasným pôrodom a možnosťami psychologickej podpory matiek na neonatologických oddeleniach. Teoretická časť obsahuje štyri kapitoly, v ktorých sú spracované témy psychického prežívania žien počas tehotenstva a pôrodu, predčasného pôrodu ako záťažovej situácie, nedonoseného novorodenca a jeho charakteristík a systému neonatologickej starostlivosti na Slovensku a v Českej republike. Výskumná časť sa venuje analýze skúseností desiatich matiek nedonosených detí pomocou metódy interpretatívna fenomenologická analýza a analýze štyroch rozhovorov odborníkov pracujúcich s matkami nedonosených detí v nemocniciach pomocou metódy vytvárania trsov. Pre výskum bol zvolený kvalitatívny prístup. K získaniu dát bola použitá metóda pološtruktúrovaného rozhovoru. Vo výskumných otázkach je pozornosť zameraná na prežívanie matiek, psychologickú a inú odbornú podporu pre matky, vplyv predčasného pôrodu na rodinu a na faktory, ktoré ovplyvňujú adaptáciu matiek na náročnú situáciu predčasného pôrodu. Z výsledkov výskumu vyplýva, že matky po predčasnom pôrode prežívajú prevažne negatívne emócie, ako strach, obavy, výčitky. Systém psychologickej podpory na neonatologických oddeleniach na Slovensku nie je dostatočný. Predčasný pôrod vplýva na chod rodiny z dôvodu časovej aj finančnej náročnosti starostlivosti o dieťa. Veľký vplyv na prijatie situácie má nízka informovanosť matiek o stave dieťaťa, prístup zdravotníckeho personálu a náročnosť starostlivosti o dieťa. Práca poskytuje pohľad na fenomén predčasného pôrodu a jeho dopady na matku a ostatných členov rodiny.

Abstract:

The diploma thesis focuses on mothers´ experience in connection with preterm birth and possibilities of psychological support for the mothers at neonatal intensive care units. The theoretical part consists of four chapters covering following themes: women mental experience during pregnancy and birth, preterm birth as a stress situation, preterm newborn and their characteristics and finally the neonatal care system in Slovakia and in the Czech Republic. The practical part focuses on the analysis of ten mothers of preterm newborns by using the interpretative phenomenological analysis technique. Also, by using the clustering method we analysed four interviews with the specialists who work with the mothers of preterm newborns in the hospitals. The practical part has been processed within the qualitative approach. Data were collected by using semi-structured interview. Research questions focus on mothers´ experience, psychological and other special mother support, the impact of preterm birth on the family and factors which influence mothers´ adaptation to a difficult situation of preterm birth. The results indicate that mothers experience negative emotions as fear, concerns and remorse after preterm birth. The system of the psychological support at neonatal intensive care units is not sufficient in Slovakia. Preterm birth influences family functioning from time and financial perspective associated with childcare. The acceptance of the situation is highly influenced by mothers´ low awareness of child state, medical staff member attitude and childcare difficulty. The diploma thesis offers a view on the preterm birth phenomenon and its impact on the mother and the rest of the family members.
