Physiotherapy after surgical treatment of fracture patelle

Abstract:

Cíl práce: přispět k řešení problematiky fyzioterapie po fraktuře pately léčenou operativně osteosyntézou. Metodika: zpracovat kazuistiku. Řízeným rozhovorem odebrat anamnézu. Metodou pozorování vyšetřit pohledem držení, trofiku, konfiguraci. Využitím technik manuální medicíny zhodnotit stav měkkých tkání, jizvy, kloubní vůle. Pomocí goniometrie vyšetřit rozsah pohybu, svalovým testem dle Jandy zhodnotit svalovou sílu, vyšetřit zkrácené struktury a tonus svalový v oblasti levého kolenního kloubu. Sledovat stereotyp chůze, pohybové stereotypy. Vyšetřit posturální reaktibilitu a stabilitu dle Koláře. Na základě těchto vyšetření stanovit a realizovat krátkodobý fyzioterapeutický plán. Výsledky:Při vstupním vyšetření byla zjištěna bolest levého kolenního kloubu, zhoršená laterální a kraniokaudální posunlivost jizvy dlouhé 10 cm, ležící na ventrální straně kolenního kloubu jdoucí vertikálně, a měkkých tkání stehna a lýtka, hypotonie v oblasti LDK (levá dolní končetina), byl omezen rozsah pohybu a kloubní vůle v daném segmentu, dále byla zjištěna snížená svalová síla, insuficience HSSP (hluboký stabilizační systém páteře) a špatný stereotyp chůze. Na podkladě výsledků vyšetření byly aplikovány techniky měkkých tkání, mobilizace, postizometrická relaxace, aktivní cvičení s fyzioballem, therabandem, oveballem, PNF (Proprioceptivní neuromuskulární facilitace), DNS (Dynamická neuromuskulární stabilizace), SMS (Senzomotorická stimulace), byla provedena korekce špatných pohybových stereotypů a chůze. Během fyzioterapeutické intervence se zvětšil rozsah pohybu do flexe levého kolenního kloubu ze 30° na 120°, zvýšila se svalová síla flexorů kolena LDK ze 3° na 4+°, extenzorů kolena LDK ze 3-° na 4+°, zlepšil se stereotyp chůze, snížila se bolestivost kolena, došlo k uvolnění jizvy laterolaterálním a kraniokaudálním směrem v celé její délce a měkkých tkání stehna a lýtka. Závěr: Výstupní kineziologický rozbor prokázal, že terapie trvající 10 týdnů od 7. 9. 2016 do 1. 11. 2016 měla pozitivní efekt, došlo ke zlepšení flexe kolenního kloubu ze 30° na 120 °, zvýšení svalové síly flexorů ze 3° na 4+° a extenzorů ze 3° na 4+° levého kolenním kloubu, k částečné úpravě stereotypu chůze, snížení bolesti. Stále přetrvává mírná hypotonie m. vastus medialis LDK. Částečně se zlepšila aktivace HSSP. Pacientce byl navržen dlouhodobý fyzioterapeutický plán. Vše nasvědčuje o prospěšnosti fyzioterapeutické intervence.

Abstract:

Goal:to contribute to dealing with the problematics of physiotherapy after a fracture of the patella treated by surgical osteosynthesis. Methodology:process casuistry. Acquire anamnesis through a directed discussion. By observation,examine posture,trophics,configuration. Evaluate the condition of soft tissues,scar,joint play with the techniques of manual medicine.Inspect the range of motion using goniometry,assess the muscle strength by Janda ´s muscle test, investigate shortened structures and muscle tonus in the area of the left knee joint. Observe gait stereotypes, movement stereotypes. Inspect postural reactiveness and stability according to Kolář. On the basis of these examinations, create and realize a short-term physiotherapeutic plam. Results:detected pain of the left knee joint,worsened lateral and craniocaudal mobility of the 10 cm long scar on the ventral side of the knee joint going vertically and soft tissues of the thing and calf, hypotonia in the area of the LLL(left lover limb), limited range of motion and joint play in the given segment, lowered muscle strength, insufficiency of DSSS(Deep stabilization system of the spine),poor gait stereotype. On the foundation of the results of examination, applied techniques of soft tissues, mobilization, post-isometric relaxation, active exercise with a physioball,theraband, overball, PNF (Proprioceptive neuromuscular facilitation), DNS (Dynamic Neuromuscular Stabilization), SMS (Sensorimotor stimulation), correction of poor movement stereotypes and walk. Throughout physiotherapeutic intervention,the range of motion to flexion of the left knee joint increased from 30°to 120°, muscle strenght the LLL knee flexors from 3°to 4+°, LLL knee extensors from3-°to 4+°, the gait stereotype improved, soreness of knee was lowered, relaxation of scar in the entirety of its length in laterolateral and craniocaudal direction and of the soft tissues of thigh and calf occurred. Concluion: Release kinesiological examination proved that a therapy taking 10 weeks 7.9 2016 to 1.11 .2016 had a positive effect. The knee flexion improved from 30° to 120°,muscle strength of the flexors of the left knee joint increased from 3°to 4+° and extensors from 3-°to 4+°, partial adjustment of gait stereotype,reduction of pain.A slight hypotonia of m. vastus medialis in the LLL remains. Activation of DSSS partially improved. Patient was recommended a long-term physiotherapeutic plan. Everything points to a benefit of physiotherapeutical intervention.
