Mobility-dependent resistance of gram-negative bacteria to colistin: resources and diversity

Abstract:

Kolistin je polymyxinové antibiotikum používané v dnešní době jako antibiotikum „poslední volby“ při léčbě infekcí vyvolaných multirezistentními gramnegativními bakteriemi. V roce 2015 byla objevena plazmidově vázaná rezistence ke kolistinu kódovaná geny mcr. Po celém světe pak následovaly studie, které popisují bakterie nesoucí geny mcr izolované od zvířat, lidí, z potravin i prostředí. Dizertační práce popisuje výskyt genů mcr a bakterií nesoucích tyto geny ve vzorcích z České republiky. Práce přispěla k rozšíření znalostí o výskytu mcr-pozitivních bakterií izolovaných z potravin (živočišného i rostlinného původu), od lidí a zvířat a dále i v povrchových a odpadních vodách České republiky. U vybraných bakteriálních izolátů byla provedena bližší genomická charakterizace, která zahrnovala lokalizaci genů mcr včetně popisu jejich umístění na plazmidu či chromozomu. Nejčastěji byly detekovány bakterie Escherichia coli nesoucí gen mcr-1, který byl lokalizován na mcr-1-typických typech plazmidů (IncX4, IncHI2 a IncI2), ale i na zřídka detekovaných plazmidech, na fágu podobném plazmidu, nebo na chromozomu. V práci byly dále popsány nálezy bakterie rodu Acinetobacter nesoucí gen mcr-4, izolát Klebsiella pneumoniae nesoucí geny mcr-1 a mcr-8 na jednom plazmidu¸ nebo izolát Escherichia coli nesoucí geny mcr-1 a mcr-3 na různých plazmidech. Výsledky této práce jsou v souladu s nálezy z celého světa, tedy že mcr-pozitivní bakterie jsou hojně rozšířené a představují hrozbu při léčbě infekcí způsobených multirezistentními bakteriemi zejména u lidí. Dizertační práce je zpracována ve formě komentovaného souboru jednotlivých publikací.

Abstract:

Colistin is a polymyxin antibiotic currently used as a last resort antibiotic for treatment of infections caused by multidrug resistant gram-negative bacteria. In 2015, plasmid borne resistance to colistin was discovered. This was followed by studies around the whole world describing bacteria carrying the mcr genes isolated from animals, human, food and the environment. The dissertation thesis describes the occurrence of the mcr genes and bacteria carrying them in samples from the Czech Republic. The thesis contributed to the extension of knowledge about the occurrence of mcr-positive bacteria isolated from food (animal and plant origin), from humans and animals and also in surface and waste waters of the Czech Republic. Selected bacterial isolates were subjected to genomic characterization which included the localization of the mcr genes on plasmid or chromosome. The most commonly detected bacteria were Escherichia coli carrying the mcr-1 gene which was localized on mcr-1-typical plasmid types (IncX4, IncHI2 and IncI2), but also on rarely detected plasmids, phage-like plasmid or on the chromosome. In this study, bacteria of genus Acinetobacter carrying the mcr-4 gene, isolate of Klebsiella pneumoniae carrying the mcr-1 and mcr-8 on one plasmid, or isolate of Escherichia coli carrying the mcr-1 and mcr-3 on different plasmids, were described. The results of this work are consistent with findings from all over the world, that mcr-positive bacteria are abundant and represent a threat in treating infections caused by multidrug resistant bacteria especially in humans. The dissertation thesis is executed in the form of an annotated collection of individual publications.
