Abstract:

Cíl: Cílem práce je přispět k řešení problematiky fyzioterapie u pacientů s radikulárním drážděním v lumbosacrální oblasti s využitím nových metod a postupů McKenzie Metodou Mechanické Diagnostiky a Terapie (MDT).Metodika: U své pacientky s iritačně zánikovým syndromem L4 l.dx. jsem provedla vstupní, výstupní i kontrolní vyšetření dle standardizovaných formulářů Mechanické Diagnostiky a Terapie McKenzie Metody. Podle výsledků vyšetření jsem pacientku zařadila do jedné ze tří skupin syndromů McKenzie Metody (derangement syndrom) a stanovila terapii v souladu s nalezenou směrovou preferencí do extenze. Toto cvičení bylo při zahájení terapie doplněno o edukaci pacientky školu zad. Pacientka také absolvovala sérii přístrojových trakcí a závěrem terapie instruktáž stabilizačního cvičení. Výsledky: Výstupní vyšetření pacientky dle kontrolního spisu McKenzie Metody? po třech měsících ukázalo maximálně možné zlepšení ve všech sledovaných parametrech -VAS score 0, Thomayerova zkouška 0, funkčnost bederní páteře při denních aktivitách dle spisu McKenzie Metody i s využitím upraveného dotazníku hodnocení bolesti dle Rollanda byla bez omezení. Při kontrolním vyšetření po šesti měsících bylo pouze mírné zhoršení VAS score, jehož příčinou mohla být zvýšená fyzická zátěž pacientky po nástupu do zaměstnání.

Abstract:

Goal: The intention is to contribute to solving problems of physiotherapy of patients with radicular irritation in lumbosacral region using new methods and approach McKenzie Method of Mechanical Diagnostics and Therapy (MDT).Methods: On my female patient suffering from extinction syndrome L4 right, I performed the entry, exit and control examination according to standardised forms of McKenzie's Mechanical Diagnostics and Therapy. I ranged the patient with one of the three syndrome groups of McKenzie's Method (derangement syndrome) in line with results of the examination and I chose the therapy in accordance with a direct preference into extension that I diagnosed. At the beginning of the therapy, the exercise was accompanied by the education of the patient about the way of the right exercising of the back. The patient also underwent series of mechanical tractions and, at the end of the therapy, she was trained in stability exercising. Results: The exit examination of the patient according to the check list of the McKenzie's Method after three months revealed maximal possible improvement in all indicated parameters: VAS score 0, Thomayer's test 0, function of a lumbar region in regards of daily activities according to McKenzie's Method file with usage of edited questionnaire of pain by Rolland, were without limitations. During control examination after six months, there was indicated just light worsening of the VAS score that could be caused by a decreased physical weight of the patient after she started to work.
