Enterobacteria resistant to carbapenems in patients in the Czech Republic

Abstract:

Carbapenems are used as antibiotics of last choice for treatment of severe nosocomial infections. In the last decade, the number of carbapenem-resistant Enterobacteriaceae (CRE) has been increasing, mainly due to the production of carbapenemases, which poses a threat to public health. These enzymes are often encoded on plasmids and tend to spread among bacteria by horizontal gene transfer. Treatment options for CRE infections are very limited. The aim of this thesis was to determine the resistance phenotype and describe the genomic properties of clinical isolates of enterobacteria resistant to carbapenems from Czech hospitals and to verify the transfer of carbapenem resistance genes by plasmids. These plasmids were identified and characterized. As part of the study, I worked with a set of 29 CRE isolates (mainly E. coli and K. pneumoniae) obtained from Czech hospitals. The minimum inhibitory concentration of isolates was tested for 24 antibiotics. The isolates showed a high incidence of resistance to beta-lactam antibiotics, aminoglycosides, quinolones, trimethoprim and tetracycline. A smaller part of the isolates showed resistance to colistin, chloramphenicol and tigecycline. Whole-genome DNA was isolated from individual isolates and then whole-genome sequencing was performed using Illumina technology. Based on the sequencing data, the sequence types (ST) of the isolates were determined, with ST11, ST147 and ST307 among the most frequently occurring STs in Klebsiella pneumomiae. The most widespread carbapenemase was NDM. OXA-48, KPC and VIM were also detected. All isolates carried more than 3 resistance genes. The most frequently found gene was blaCTX-M-15 encoding broad-spectrum beta-lactamases, or genes encoding resistance to aminoglycosides, sulfonamides and tetracycline. IncF-type and IncR plasmids were the most common among the isolates. After evaluating the conjugation experiment and comparing it with the results from long-read sequencing using ONT technology, it was found that IncL/M plasmids play an important role in the transfer of blaOXA-48-like genes. The blaNDM-1 gene is usually associated with transmission on IncF-type plasmids and the blaKPC-2 gene can be carried by IncR plasmid. These plasmids often carry resistance genes to several antibiotics. The results obtained in the diploma thesis prove that plasmid-encoded carbapenemases play a key role in the spread of resistance to carbapenems in hospital environment. It has also been proven that CREs often exhibit multidrug resistance, making the treatment of nosocomial infections even more difficult.

Karbapenemy jsou používány jako antibiotika poslední volby k léčbě závažných nozokomiálních infekcí. V posledním desetiletí narůstá počet enterobakterií rezistentních ke karbapenemům (CRE), a to hlavně kvůli produkci karbapenemáz, což představuje hrozbu pro veřejné zdraví. Tyto enzymy bývají často kódovány na plazmidech a mají tendenci šířit se mezi bakteriemi horizontálním přenosem. Možnosti léčby infekcí způsobených CRE jsou velmi omezené. Cílem této práce bylo určit fenotyp rezistence a popsat genomické vlastnosti izolátů enterobakterií rezistentních ke karbapenemům získaných z pacientů v České republice, a dále ověřit přenos genů rezistence ke karbapenemům pomocí plazmidů. Tyto plazmidy byly následně identifikovány a typizovány. V rámci studie bylo pracováno se souborem 29 izolátů CRE (převážně E. coli a K. pneumoniae) získaných z českých nemocnic. U izolátů byla testována minimální inhibiční koncentrace k 24 antibiotikům. Byl zaznamenán vysoký výskyt rezistence k beta-laktamovým antibiotikům, aminoglykosidům, chinolonům, trimethoprimu a tetracyklinu. Menší část izolátů vykazovala rezistenci ke kolistinu, chloramfenikolu a tigecyklinu. U jednotlivých izolátů byla izolována celogenomová DNA a následně bylo provedeno celogenomové sekvenování technologií Illumina. Na základě sekvenačních dat byly určeny sekvenční typy (ST) izolátů. Mezi nejčastěji se vyskytující ST v případě Klebsiella pneumomiae patřily ST11, ST147 a ST307. Nejrozšířenější karbapenemázou byla NDM, detekovány byly i OXA-48, KPC a VIM. Všechny izoláty nesly více než 3 geny rezistence. Často nalezeným genem byl blaCTX-M-15 kódující širokospektré beta-laktamázy, nebo geny kódující rezistenci k aminoglykosidům, sulfonamidům a tetracyklinu. U zkoumaných izolátů se nejvíce vyskytovaly plazmidy typu IncF a IncR. Po vyhodnocení konjugačního experimentu a srovnání s výsledky ze sekvenování dlouhých čtení technologií ONT bylo zjištěno, že v přenosu blaOXA-48-like hrají důležitou roli plazmidy IncL/M, gen blaNDM-1 bývá spojován s přenosem na IncF-type plazmidech a blaKPC-2 může být přenášen plazmidem IncR. Tyto plazmidy mnohdy obsahují geny rezistence k více antibiotikům. Výsledky získané v rámci diplomové práce dokazují, že plazmidem kódované karbapenemázy hrají klíčovou roli v šíření rezistence ke karbapenemům v nemocničním prostředí. Také bylo prokázáno, že CRE často vykazují multirezistenci, čímž ještě více ztěžují léčbu nozokomiálních infekcí.
