New possibilities for combination therapy in the model of pediatric solid tumors

Abstract:

Terapie založená na kombinaci různých antineoplastických sloučenin nadále zůstává základním principem léčby v onkologii. Nízko-dávková metronomická kombinovaná chemoterapie s potenciálem zacílit nejen na maligní buňky, ale také na nádorové mikroprostředí, představuje méně toxickou alternativu k standardní vysoko-dávkové chemoterapii. Tato dizertační práce je zaměřena na vybrané mechanizmy spolupůsobení léčiv využívaných v rámci metronomických protokolů při léčbě dětských solidních nádorů. Na základě mírně odlišných témat byly dosažené výsledky rozděleny do dvou částí. Cílem první části bylo popsat vliv nádorového mikroprostředí na účinky terapie pomocí kyseliny all-trans retinové (ATRA) v kombinaci s kyselinou kávovou nebo s celecoxibem na modelu sbírkových buněčných linií, které byly derivovány z neuroblastomu. Analýzy prokázaly, že kyselé pH (6.8) je u obou testovaných linií spojeno s rezistencí k aplikovaným sloučeninám, zatímco deprivace glukózy (1 mM) je spojena se zvýšeným účinkem celecoxibu. Hypoxie (1% O2) ovlivnila citlivost vybraných buněčných linií k aplikované léčbě zcela rozdílně: buňky linie SH-SY5Y se staly citlivější k účinku ATRA, zatímco buňky linie SK-N-BE(2) vykazovaly rezistenci ke všem aplikovaným sloučeninám. Na základě této odlišnosti byly další analýzy zaměřeny na hledání možných markerů rezistence k ATRA za hypoxických podmínek, konkrétně byla sledována přítomnost amplifikace MYCN, případně vysoká exprese SHC3. Druhá část dizertační práce byla zaměřena na detailní analýzu možných synergických účinků mezi běžnými cytotoxickými léčivy a novým druhem thiosemikarbazonů na modelu buněčných linii derivovaných z nejčastějších typů dětských solidních nádorů. Stanovení kombinačních indexů potvrdilo synergické působení pro každou testovanou kombinaci sloučenin. V části II.A byly hodnoceny interakce thiosemikarbazonů s celecoxibem, temozolomidem nebo etoposidem. Odhalené synergie mezi sloučeninami byly specifické pro jednotlivé buněčné linie. V části II.B byly analyzovány interakce thiosemikarbazonů se třemi vybranými léčivy ze skupiny antracyklinů a synergie byla potvrzena u všech kombinací, ve všech pěti testovaných liniích. Následně, po přidání podrobnějších analýz na molekulární úrovni, byly navrženy různé mechanismy interakcí pro všechny kombinace sloučenin a taktéž nejúčinnější způsob aplikace zvolené kombinované léčby.

Abstract:

Combination treatment with various antineoplastic compounds remains the cornerstone of therapeutic modalities in oncology. As a less toxic alternative to high-dose therapy, low-dose metronomic regimens have been designed, some of them with the potential to influence not only malignant cells but also the tumor microenvironment. This Ph.D. thesis focuses on the selected mechanisms of combination therapies in cell lines derived from the most frequent types of pediatric solid tumors. Regarding the slightly different topics in the thesis, the results are subdivided into two parts. Part I aimed to describe the influence of the tumor microenvironment on combined treatment with all-trans retinoic acid (ATRA) and caffeic acid or celecoxib of neuroblastoma cell lines. The analyses revealed an acidic pH (6.8) caused resistance to the applied compounds, whereas glucose deprivation (1 mM) sensitized the cells to celecoxib. Hypoxia (1 % O2) influenced the selected cell lines differently: SH-SY5Y cells responded markedly to ATRA treatment, while SK-N-BE(2) cells showed resistance to all the applied compounds. Therefore, the putative markers that were associated with resistance to ATRA under hypoxic conditions were investigated, and the amplification of MYCN and high expression of SHC3 were suggested. Part II aimed to analyze, in detail, the possible synergistic effects between cytotoxic drugs routinely used in combination therapies and novel potent thiosemicarbazones in cell lines derived from pediatric solid tumors. Using the calculation of combination indexes based on dose-dependent growth curves, synergistic action was confirmed for each combination of compounds. In Part II.A, the interactions among celecoxib, temozolomide, etoposide and thiosemicarbazones were evaluated, revealing that the synergies seemed to be cell line specific. In Part II.B, the interactions between three selected drugs from the anthracycline class and thiosemicarbazones were evaluated, and synergy was confirmed for all the compound combinations in all five tested cell lines. Additionally, several different mechanisms of interactions for all the compound combinations were suggested as well as the most effective design of combination treatment.
