Occupational therapy in patients with bilateral amputation of the lower extremities in older age

Abstract:

V poslední době přibývá více pacientů s diabetes mellitus, což je jedna z nejčastějších příčin amputací. Proto je potřeba se o tomto tématu zmiňovat, především o možnostech řešení situací, do kterých se pacienti po amputaci dostávají. Cílem mé bakalářské práce je poukázat na to, že ergoterapie má pozitivní vliv u pacientů ve starším věku po oboustranné amputaci dolních končetin. Dalším z cílů je dokázat, že ergoterapie má význam na cestě ke zlepšení soběstačnosti a pozitivně ovlivňuje také psychiku jedince. Práce je rozdělena na teoretickou a praktickou část. V teoretické části se zaměřuji na představení klíčových pojmů, jako je stáří a problematika stáří. Zmiňuji informace o amputacích, včetně jejich etiologie a komplikací, péči po amputaci. Dále se věnuji ergoterapii a rehabilitaci v souvislosti s osobami po amputaci. Další kapitolu tvoří ortopedická protetika spolu s kompenzačními pomůckami a invalidními vozíky. Praktická část je zpracována formou dvou kazuistik, které obsahují anamnézy, vyšetření, testování a shrnutí hlavní problematiky klientů. V další části je zpracována realizace ergoterapeutického plánu a doporučení pro dlouhodobý ergoterapeutický plán. Výsledky ukazují vliv kompenzačních pomůcek na soběstačnost klientů, pozitivní ovlivnění psychiky díky ergoterapii ale i negativní dopad na náladu klienta po nezdařilém plánu realizace jízdy na elektrickém vozíku.

Abstract:

Recently there have been a rise of patients with diabetes mellitus which is one of the most common causes of amputations. Therefore, it is necessary to mention this topic, especially the possibilities of solving situations that patients get into after the amputation. The main aim of my bachelor thesis is to point out that occupational therapy has a positive effect in elderly patients after bilateral amputation of the lower limbs. Another goal is to prove that occupational therapy is an important part on the way to improve self-sufficiency and has a positive effect on the individual's psyche. The thesis is divided into a theoretical and a practical part. In the theoretical part I focus on introducing the key terms such as an old age and the issue of old age. I mention information about amputations, including its etiology and complications and care after the amputation. I also deal with occupational therapy and rehabilitation in connection with people after the amputation. The next chapter consists of orthopedic prosthetics along with compensatory aids and wheelchairs. The practical part is processed in the form of two case studies, which include anamnesis, examination, testing and a summary of client's main issues. The next part deals with implementation of the occupational therapy plan and recommendations for a long-term occupational therapy plan. The results show the effect of compensatory aids on the client's self-sufficiency, a positive effect on the psyche due to occupational therapy, but also a negative impact on the client's mood after a failed plan to drive an electric wheelchair.
