Methods of treatment of burns in children

Abstract:

Cílem přehledové bakalářské práce bylo sumarizovat aktuální výsledky metod ošetření popálenin u dětských pacientů. Na základě stanovených výzkumných otázek byly pro sběr dat použity vybrané elektronické databáze v českých a zahraničních zdrojích MEDLINE Complete, PubMed, CINAHL Plus with Fullext, Science Direct, ProQuest, BiblioMedica, Medvik. Nalezené relevantní práce byly důkladně vyhodnoceny dle zvolených kritérií tak, aby splnily účel přehledové studie. Extrakce, analýza a syntéza dat proběhla na základě stanovených dílčích cílů práce. Analyzováno bylo 11 studií. Dohledané studie zkoumaly ošetření popálenin v rámci poskytování první pomoci chlazením a porovnávaly rozdílné postupy chlazení. Dále studie analyzovaly, zda v rámci poskytování první pomoci chlazením dochází k určitému stupni analgetizace. Všechny články zahrnovaly výsledky výzkumu zaměřeného na ošetření popálenin v přednemocniční péči anebo při poskytování první pomoci u popálenin dětských pacientů v ambulancích, či na pohotovostních příjmech. Bylo zjištěno, že první pomoc chlazením studenou tekoucí vodou 20 minut u popáleninového traumatu u dětských pacientů měla vliv na rychlejší hojení popálenin a snížení komplikací spojených s popáleninovým traumatem a celkově kratší hospitalizaci. Dále bylo zjištěno, že chlazení aplikováno na popálenou kůži a tkáň mělo analgetický účinek, přesto bylo vhodné tuto nefarmakologickou metodu kombinovat s dostupným farmakologickým analgetikem. Přestože se autoři analyzovaných studií jednotně neshodli na trvání chlazení a cílové teplotě studené tekoucí vody, chlazení vodou přineslo jasné výsledky. U hodnocení bolesti měly studie značné mezery ve velikosti hodnoceného vzorku. Přesto měla metoda chlazením pomocí studené tekoucí vody při poskytování první pomoci u popáleninového traumatu u dětského pacienta význam v obou bodech.

Abstract:

The aim of the bachelor thesis was to summarise the current results of burn treatment methods in paediatric patients. Based on the research questions, selected electronic databases from Czech and foreign sources MEDLINE Complete, PubMed, CINAHL Plus with Fullext, Science Direct, ProQuest, BiblioMedica, Medvik were used for data collection. The relevant papers found were thoroughly evaluated according to the selected criteria to fulfil the purpose of the review study. Data extraction, analysis and synthesis were performed based on the stated sub-objectives of the study. 11 studies were analysed. The retrieved studies investigated the treatment of burns in first aid by cooling and compared different cooling methods. In addition, the studies analysed whether some degree of analgesia occurs during the provision of first aid by cooling. All the articles included the results of trials that focused on the treatment of burns in the prehospital setting and/or first aid for burns in paediatric patients in outpatient clinics or emergency departments. It was found that first aid treatment of burn trauma in paediatric patients by cooling with cold running water for 20 minutes resulted in faster healing of burns, fewer complications associated with burn trauma and a shorter overall hospital stay. Cooling the burned skin and tissue was also found to have an analgesic effect, but it was appropriate to combine this non-pharmacological method with an available pharmacological analgesic. Although the authors of the trials analysed did not agree on the duration of cooling and the target temperature of cold running water, water cooling produced clear results. There were significant differences in the sample sizes of the trials for pain assessment. The cold running water cooling method of first aid for burn trauma in a paediatric patient was significant on both counts.
