Physioterapy after total hip replacement

Abstract:

Cíl: Cílem této bakalářské práce je přispět k řešení problematiky fyzioterapie po totální aloplastice kyčelního kloubu v období hospitalizace. Dalším cílem této bakalářské práce je přispět k řešení problematiky volby optimálních fyzioterapeutických postupů, konceptů a metod u vybraného pacienta po totální aloplastice kyčelního kloubu. Metodika: V bakalářské práci je zpracována kazuistika ženy ve věku 68 let s primární diagnózou coxarthrosis lateris dextrí a indikací k totální aloplastice kyčelního kloubu. Bylo provedeno vstupní vyšetření a stanoven krátkodobý fyzioterapeutický plán u vybrané pacientky s totální aloplastikou kyčelního kloubu. Dále bylo provedeno kontrolní vyšetření se zaměřením na sílu stabilizátorů kyčelního kloubu a to 5., 9., a 15. den po operaci a 2 měsíce po operaci. Nakonec bylo provedeno výstupní vyšetření a stanoven dlouhodobý fyzioterapeutický plán u pacientky po totální aloplastice kyčelního kloubu. Mezi fyzioterapeutické metody a postupy jsem zařadila: CG, DG, pasivní pohyby, aktivní pohyby, techniky měkkých tkání, míčkovou facilitaci, PIR, RS, PNF, kinesiotaping, mezi pomůcky jsem zařadila overball a theraband s přihlédnutím k Brügger konceptu. Výsledky: Výstupní kineziologický rozbor prokázal zlepšení a to hned v několika parametrech: zvětšení rozsahů pohybů pravého kyčelního kloubu do flexe, extenze, abdukce a vnitřní rotace, nárůst svalové síly operované dolní končetiny- zvětšení v průměru o 1 stupeň, eliminace otoku a patologické bariéry v oblasti inquinální fascie vpravo, navození správného stereotypu pohybu do extenze a abdukce kyčelního kloubu bilaterálně, snížení stupně zkrácení svalových skupin ve všech třech zjištěných případech, navozen správný stereotyp chůze.

Abstract:

Purpose: the Aim of this thesis is to contribute to the solution of the problems of physiotherapy after total hip replacement in a period of hospitalization. Another aim of this thesis is to contribute to solving problems of choice of optimal physiotherapy practice, concepts and methods at the selected patient after a total hip replacement. Methodology: In my bachelor thesis is processed by a case report a woman aged 68 years old with a primary diagnosis of coxarthrosis lateris dextrí and indications for total hip replacement. Was done initial examination and determined the short-term physiotherapist's plan for selected patients with total hip replacement. Furthermore, it was carried out a control examination with a focus on the strength of the stabilizers of the hip joint and it a 5., 9., and 15. the day after operation and 2 months after the operation. Finally, it was done the exit examination and establish a long-term physiotherapist's plan for the patient after total hip replacement. Among the physiotherapeutics methods and procedures I have included: CG, DG, passive movements, active movements, soft tissue techniques, balls facilitation, PIR, RS, PNF, kinesiotaping, between the aids I have included a overballs and theraband with regard to Brügger concept. Results: An output assesment showed an improvement in many criteria: an increase in range of movement on the operated leg in flextion, extension, abduction and inner rotation, a gain in a muscle strenght on the operated leg- an increase in average of one degree,a removal of a swelling, patological barrier of an inguinal fascia, correct movement stereotypes of hip extension and abduction bilaterally, a lowered level of a shortened muscles in all three assesed cases, correct gait movement.
