Posouzení hodnoty předoperační hladiny nádorových markerů CEA a CA 19-9 kolorektálního karcinomu v korelaci s lokalizací nádoru v tlustém střevě a stadiem onemocnění

Association of preoperative levels of colorectal tumor markers CEA and CA 19-9 with the staging of the disease and tumor location

Abstract:

Úvod: Kolorektální karcinom představuje celosvětově vážný zdravotnický i ekonomický problém. Ve vyspělých zemích je nejčastější malignitou gastrointestinálního traktu a jeho incidence, až na výjimky, stále stoupá. Jedinou kurativní léčbou stále zůstává léčba chirurgická, která je indikována na základě pečlivě provedeného předoperačního stagingu onemocnění. Součástí předoperačního vyšetření by měl být také odběr nádorových markerů. Význam a uplatnění nádorových markerů je však stále diskutován. Cílem této práce je zhodnotit předoperační hodnotu nádorových markerů CEA a CA 19 9 kolorektálního karcinomu s ohledem na lokalizaci nádoru v tlustém střevě a stadium onemocnění a na základě těchto nálezů poté posoudit přínos těchto markerů pro diagnostický staging. Metody: V klinické části disertační práce byli retrospektivně hodnoceni pacienti, kteří byli operováni pro kolorektální karcinom na Chirurgické klinice Fakultní nemocnice v Ostravě v letech 2015-2018, a kterým byly předoperačně odebrány nádorové markery CEA a CA 19 9. Získaná data byla následně statisticky zpracována pomocí statistického softwaru R - project. K testování byl použit Kruskal-Wallisův test s post-hoc analýzou (test Dunnové). Výsledky: Do souboru bylo zařazeno celkem 155 pacientů, 96 (62 %) mužů a 59 (38 %) žen. Nejvíce pacientů, 74 (48 %), mělo nádor lokalizován v rektu. Nejméně pacientů 18 (12 %) bylo zachyceno ve stadiu IV. CEA byl odebrán u všech 155 pacientů, CA 19 9 u 105 pacientů. Medián CEA byl roven 3 (rozsah 0.34-1104.25), medián CA 19 9 byl roven 12 (rozsah 0.18-840.00). Pomocí statistické analýzy byla prokázána existence statisticky významné závislosti mezi mediány markeru CEA a stadiem onemocnění (p=0,016), kdy stadia I, II a III (mediány 2, 3, 2) tvoří homogenní skupinu, od nichž se stadium IV (medián 13) významně odlišuje. Naopak nebyla prokázána existence statisticky významné závislosti mezi mediány markeru CA 19 9 a stadiem onemocnění (p=0,343). Ani u jednoho z markerů nebyla prokázána statisticky významná závislost na hladině p=0,05 mezi těmito markery a lokalizací (CEA: p=0,276; CA 19 9: p=0,505). Závěr: Práce prokázala statisticky významnou závislost mezi nádorovým markerem CEA a stadiem onemocnění, zároveň však nepotvrdila vztah těchto markerů k lokalizaci tumoru. Studie tedy potvrdila závěry již dříve proběhlých studií, kdy z vyšších hodnot CEA lze usuzovat na nález pokročilejších forem kolorektálního karcinomu, a tedy i na horší prognózu. Umístění nádoru v jednotlivých částech tkustého střeva nebo v konečníku z předoperačních hodnot těchto nádorových markerů predikovat nelze.

Abstract:

Introduction: Colorectal cancer is a serious health and economic problem worldwide. In developed countries, it is the most common malignancy of the gastrointestinal tract and its incidence, with few exceptions, keeps increasing. Surgical treatment remains the only curative treatment, which is indicated on the basis of careful preoperative staging of the disease in a particular patient. The preoperative evaluation should also include the collection of tumour markers. However, the relevance and application of tumour markers are still much discussed. The aim of this thesis was to evaluate the preoperative value of tumor markers CEA and CA 19-9 in colorectal cancer with respect to the tumor location in the colon and disease stage, and, based on these findings, to assess their contribution to diagnostic staging. Methods: In the clinical part of the thesis, retrospective evaluation of records of patients who were surgically treated for colorectal carcinoma at the Department of Surgery of the University Hospital Ostrava in 2015-2018 and in whom tumour markers CEA and CA 19-9 were collected preoperatively was performed. The acquired data were statistically evaluated in the R software using the Kruskal-Wallis test with post-hoc analysis (Dunn's test). Results: In all, 155 patients were included in the study, 96 of which (62 %) were male and 59 (38 %) female. In most patients (74; 48 %), the tumor was located in the rectum. The lowest number of patients, 18 (12 %), were in stage IV. CEA was collected in all 155 patients, CA 19-9 was collected in 105 patients. Median CEA was equal to 3 (range 0.34-1104.25), median CA 19-9 was equal to 12 (0.18-840.00). Statistical analysis showed a statistically significant relationship between median CEA markers and disease stage (p=0.016), with stages I, II, and III (median 2, 3, 2) forming a homogeneous group from which stage IV (median 13) significantly differed. On the other hand, there was no statistically significant association between the median CA 19-9 marker and disease stage (p=0.343). No statistically significant relationship at the level of p=0.05 was found between either marker and localization (CEA: p=0.276; CA 19-9: p=0.505). Conclusions: The study showed a statistically significant relationship between the tumor marker CEA and disease stage but did not confirm the relationship of either of these markers with tumor location. Thus, the study confirmed the findings of previous studies, where higher CEA values suggest the presence of more advanced forms of colorectal cancer and, therefore, a worse prognosis. The preoperative values of these tumour markers cannot predict the location of the tumour in individual parts of the colon or rectum.
