Predicting inpatient costs using regression tree models

Abstract:

Tato bakalářská práce se věnuje problematice modelování nákladů na hospitalizační péči. Poskytuje popis klasifikačních systémů typu DRG (Diagnosis Related Groups), jejichž principem je klasifikace pacientů do nákladově homogenních skupin na základě klinických a demografických faktorů. V teoretické části je objasněn matematický aparát vybraných metod regresních stromů a je demonstrována dosavadní role regresních stromů při analýze nákladnosti hospitalizační péče nejenom v souvislosti DRG systémů. S využitím metod CART (Classification and Regression Trees) a CHAID (Chi-square Automatic Interaction Detection) regresních stromů bylo vytvořeno klasifikační schéma pacientů hospitalizovaných s rakovinou kolorekta, které zohledňuje zejména provedené zdravotní výkony a faktory spojené se závažností případu. Na základě výsledků lze konstatovat, že regresní stromy mohou být nápomocny při tvorbě a kultivaci klasifikačních systémů pacientů, jakožto podkladu pro úhradu jejich hospitalizace.

Abstract:

This bachelor thesis deals with predicting inpatient costs using regression trees. It provides a description of DRG (Diagnosis Related Groups) classification systems, principle of which is to classify patients into cost-homogeneous groups based on clinical and demographic factors. In the theoretical part, the mathematical apparatus of selected methods of regression trees is clarified and the role of regression trees in connection with the analysis of inpatient costs not only in the context of DRG systems is demonstrated. With the use of CART (Classification and Regression Trees) and CHAID (Chi-square Automatic Interaction Detection), methods of regression trees, a classification scheme of patients hospitalized with colorectal cancer was created. This scheme takes into account performed medical procedures and factors related to the case severity. Based on the results, regression trees can be helpful in formation and cultivation of patient classification systems, that can serve as a basis for reimbursement of inpatient care.
