Quality of Life of Elderly Patients with Peripheral Arterial Disease

Abstract:

Cíl: Hlavním cílem diplomové práce bylo zjistit kvalitu života seniorů s ischemickou chorobou dolních končetin. Záměrem bylo vypátrat, které dimenze kvality života jsou nejvíce ohroženy. Zvolenými dílčími cíli jsme sledovali vliv pohlaví, věku a stádia onemocnění na kvalitu života. Získané výsledky jsme dále porovnali s daty běžné populace. Metodika: Výzkumný soubor tvořilo 106 seniorů s ischemickou chorobou dolních končetin, hospitalizovaných ve zdravotnickém zařízení, v době od 1. 1. 2019 do 31. 8. 2019. Pro sběr dat byl použit standardizovaný dotazník WHOQOL-BREF a EQ-5D-5L, které byly navíc v úvodní části rozšířeny o demografické údaje. Výsledky: Nejvíc seniorů uvádělo potíže s mobilitou a bolestí (93,4 %). Nejnižší kvalita života byla nalezena v doméně fyzické zdraví (x? = 46,8?18, 93). Při porovnání výsledků našich dat s daty běžné populace, dosáhli senioři s ischemickou chorobou dolních končetin statisticky významně nižších hodnot (p<0,001) v kvalitě života, spokojenosti se zdravím, v doménách fyzické zdraví, prožívání a sociálních vztazích. Demografické údaje - věk ani pohlaví neměly vliv na kvalitu života. S vyšším stádiem onemocnění, kvalita života klesala, nejméně byli senioři spokojeni se zdravotním stavem (p=0,0047) a také s fyzickým zdravím (p=0,0001). Závěr: Z našich výsledků vyplývá, že senioři s ischemickou chorobou dolních končetin jsou ohroženi nízkou kvalitu života. Proto je důležité zaměřit se na zlepšení péče a self-péče v oblastech mobility, denních aktivit seniorů a léčbu bolesti. Důležitá je také psychická podpora. Pro udržení sociálních kontaktů je nutné vytvořit vhodné podmínky.

Abstract:

Aim: The main aim of the master thesis was to determine the quality of life of elderly with peripheral arterial disease. The intention was to find out which dimensions of quality of life are at biggest risk. We monitored the influence of gender, age and stage of the disease on the quality of life through the selected sub-objectives. We further compared the obtained results with the data of the general population. Methodology: The research group consisted of 106 elderly with peripheral arterial disease, hospitalized in a medical facility, in period from 1 January 2019 to 31 August 2019. A standardized WHOQOL-BREF and EQ-5D-5L questionnaire was used for data collection. which were additionally expanded by demographic data in the introductory part. Results: Most elderly reported mobility problems and pain (93,4 %). The lowest quality of life was found in the domain of physical health (x? = 46.8 ? 18, 93). When comparing the results of our data with data from the general population, elderly with periheral arterial disease achieved statistically significantly lower values (p <0.001) in quality of life, health satisfaction, in the domains of physical health, experience and social relationships. Demographics ? age and gender did not affect quality of life. With a upper stage of the disease, the quality of life decreased, the elderly were the least satisfied with the state of health (p = 0.0047) and also with physical health (p = 0.0001). Conclusion: Our results show that elderly with peripheral arterial disease are at risk for poor quality of life. Therefore, it is important to focus on improving care and self-care in the areas of mobility, daily activities of elderly and pain management. Psychological support is also important. To maintain social contacts, it is necessary to create suitable conditions.
