Vliv terapeutické inhibice BCR signalizace na genovou expresi u B buněčných malignit a vývoj nové cílené léčby

The effect of therapeutic inhibition of BCR signalling on gene expression in B cell malignancies and the development of new targeted therapy

Abstract:

Oficiální zadání disertační práce bylo následující. Chronická lymfocytární leukémie (CLL) tvoří asi třetinu všech případů leukémií v západním světě a jedná se o stále nevyléčitelné onemocnění. Jednou z klíčových hypotéz vysvětlujících vznik a terapeutickou rezistenci tohoto onemocnění je vliv mikroprostředí kostní dřeně a lymfatických uzlin. Hromadící se poznatky o patofyziologii CLL a dalších B buněčných malignit vedly k zavedení léčiv inhibujících interakce v mikroprostředí včetně použití inhibitorů adheze a signalizace přes B buněčný receptor (BCR), který je zodpovědný za přežívání a proliferaci maligních B lymfocytů. Použití inhibitorů signalizace přes B-buněčný receptor (BCR) má slibné terapeutické výsledky u B-buněčných malignit, ale nejsou dobře známy mechanismy jejich působení a molekulární dráhy důležité pro vznik rezistence a volbu vhodné kombinační léčby. V projektu budeme studovat expresi protein-kódujících genů po aplikaci BCR-inhibitorů v maligních buňkách pacientů s chronickou lymfatickou leukémií. V předběžných analýzách byly identifikovány mRNA ovlivněné BCR-inhibicí a bylo zjištěno, že BCR-asociované mRNA mohou být využity jako biomarkery, což bude dále testováno na velkých souborech pacientů s CLL. Vhodným kandidátem pro toto studium je povrchová molekula CD20, jejíž hladiny se mění po terapeutickém použití BCR-inhibitorů, stejně jako během interakcí v mikroprostředí CLL buněk. Z tohoto důvodu bude projekt také zaměřen na studium vzájemných interakcí buněk mikroprostředí s B lymfocyty a jejich vliv na expresi protein-kódujících genů.

Abstract:

Official assigning the theme of the PhD thesis. Chronic lymphocytic leukemia (CLL) represents approximately one third of all types of leukemias in the Western word and it is still an incurable disease. A tumor microenvironment of bone marrow and lymph nodes seems to have a key impact on an origin and therapeutical resistance of the disease. Accumulating knowledge about the pathophysiology of CLL and other B cell malignancies immediately leads to the introduction of drugs inhibiting the microenvironmental interactions including the inhibitors of adhesion and signalling through the B cell receptor (BCR), which is responsible for the survival and prolifer-ation of malignant B lymphocytes. Therapeutically used BCR inhibitors bringing promising therapeutic results in B-cell malignancies, but the mechanisms of their action and the molecular pathways important for the emergence of resistance and the choice of suitable combination therapy are not well elucidated. In the project, we will study the expression of protein-coding genes after the application of BCR-inhibitors in malignant cells of patients with chronic lymphocytic leukemia. In pre-liminary analyses, mRNAs affected by BCR-inhibition were identified and it was found that these mRNAs can be used as biomarkers, which will be tested in large cohorts of CLL patients. The surface molecule CD20 is suitable candidate for this study as its levels are changed after the therapeutic use of BCR-inhibitors and during microenvironmental interactions of CLL cells. For this reason, the project will be also focused on the study of mutual microenvironmental interactions of the B lymphocytes and their influence on the expression of protein-coding genes.
