Neuropsychic stress of families caring for a child with diabetes mellitus

Abstract:

Cíl: Cílem práce bylo posoudit zvládání zátěže v rodinách pečujících o dítě s diabetes mellitus I. typu. Dále jsme zjišťovali, zda a do jaké míry ovlivňuje psychická zátěž jednotlivých pečujících funkci celé rodiny. Metodika: Výzkumný soubor tvořilo 114 rodičů diabetických dětí, kteří souhlasili s vyplněním anonymního standardizovaného dotazníku. Dotazníkové šetření (září ? říjen 2012) bylo realizováno standardizovanými hodnotícími nástroji: Family hardiness index, Apgarové skóre rodin a Burn-out Measure. Dále byla použita modifikace nestandardizovaného dotazníku ? Hodnocení zátěže pečovatele (Jedlinská, 2009). Výsledky: Ze statistického měření vyplývá, že odolnost rodin s diabetickými dětmi souvisí s funkčností rodin. 70 % rodin ze zkoumaného vzorku můžeme označit za funkční. Byla prokázána výrazná zátěž pečovatelů, kterou respondenti především pociťovali velkou mírou odpovědnosti, ale i pracovním přetížením, časovým stresem, neovlivnitelností situace, nesvobodou, bezmocí a dlouhodobým napětím. Tuto skutečnost potvrdila průměrná hodnota BM indexu 3,31. Pomoc rodiny a zdravotníků hodnotili rodiče jako velmi dobrou, ovšem podporu státu a společnosti označili za nedostatečnou. Snížená odolnost byla prokázána u rodin s dětmi, které měly diabetes mellitus déle jak 5 let. Dále bylo zjištěno, že velké psychické zatížení pečovatele má negativní vliv na zvládání zátěže rodiny a tudíž i na její funkčnost. Závěr: Péče o děti s diabetes mellitus představuje pro rodiny velkou psychickou zátěž. Podstata zvládání zátěže rodiny v tíživé životní situaci spočívá především v otevřené a přímé komunikaci, zdravé soudržnosti a schopnosti adaptovat se na nové podmínky. Úkolem zdravotníků je poskytnout rodině podporu a podat pomocnou ruku při zvládání tíživých situací spojených s onemocněním dítěte. Nové možnosti spatřujeme zvláště v rozvoji komunitní péče v oblasti pediatrie a zřízením pozice školní sestry.

Abstract:

Aim: The aim of this work was to assess how well the burden is managed in the families taking care of a child with diabetes mellitus I. type. The further exploration was to find out if and to what extend the burden imposed on each individual family member taking care of such child affects the function of the whole family. Methodology: Research group consisted of 114 parents of diabetic children who agreed to fill in standardised anonymous questionnaire. The questionnaire survey took place in September and October 2012 and was carried out with standardised evaluation tools: Family hardiness index, Apgars score of family and Burn-out Measure. Modification of the non-standardised questionnaire ? Evaluation of the burden on caretaker (Jedlinská, 2009) was also used. Results: The statistic measure shows that the resistance to burden for families with diabetic children is related with the functionality of such families. 70 % of the families from the investigated sample can be called functional. A significant burden on the caretaker has been proved out and has relation to feeling of great amount of responsibility but also work overload, time stress, inability to influence the situation, absence of freedom, helplessness and long lasting tension. This fact was confirmed by average value of BM index of 3,31. The help of family and medics was evaluated by parents as very good. The support of the state and society was evaluated as inadequate. Reduced resistance has been proved out by families with children who have had diabetes mellitus Langer than 5 years. Other findings show that the great psychical burden on the caretaker has a negative impact on managing all requirements on the family and thus on its functionality. Conclusion: The care for children with diabetes mellitus mean great psychic burden for families. The core of managing the burden on families in oppressive life situation lie in an open and direct communication, good compactness and the ability to adapt to new conditions. The task for the medics is to provide support to such families and give help when managing difficult situations related to a child that is taken ill. New possibilities can be seen in the development of community care in the area of paediatrics and also setting up the position of school nurse.
