Development of problems based on daily records during Dialectic Behavioral Therapy in Patients with Borderline Personality Disorder

Abstract:

The main goal of this thesis was to explore the development of difficulties in patients with the borderline personality disorder based on daily records during dialectical behavioural therapy. The examined data come from an extensive longitudinal research of the Psychiatric Clinic of the University of Brno. The theoretical part is devoted to the description of the borderline personality disorder (BPD) and dialectical behaviour therapy (DBT). The research sample of the DBT program consisted of 11 participants (11 women) and their age ranged from 20 to 35 years (M = 26 years; SD = 5.27 years). The main research methods were diary cards in which DBT program participants filled out the amount of perceived HPO-related symptoms each day, as well as the use of the different DBT skills on that day. In addition to the diary cards, online self-reported questionnaires were used for the research during four measures - at the beginning of therapy, mid-therapy, at the end of therapy, and six months after DBT therapy. Those questionnaires measured participants' borderline symptoms, impulsivity, emotion dysregulation, dissociation, anxiety, depression, and mindfulness. Results from the analysis of the diary cards show a statistically significant reduction in the urge to self-harm and the number of self-injuries compared to the first half of the DBT program. In contrast, the number of the used DBT skills increased significantly compared to the first half of the program. According to the analysis of the self-reported questionnaires, there was a statistically significant reduction in participants' borderline symptoms, emotional dysregulation, depression, and a significant increase in mindfulness. The presented study is rather exploratory due to the insufficient psychometric properties of the diary cards. Nevertheless, our results have contributed to expand the knowledge in the field of research on the effect of DBT therapy in BPD patients.

Abstract:

Hlavným cieľom tejto práce bolo preskúmať vývoj ťažkostí u pacientov s hraničnou poruchou osobnosti na základe denných záznamov počas dialektickej behaviorálnej terapie. Skúmané dáta pochádzajú z rozsiahleho longitudinálneho výskumu Psychiatrickej kliniky Fakultnej nemocnice Brno. Teoretická časť sa venuje popisu hraničnej poruchy osobnosti (HPO), a tiež dialektickej behaviorálnej terapie (DBT). Výskumnú vzorku DBT programu tvorilo 11 účastníčok (11 žien) a ich vek sa pohyboval v rozmedzí 20 až 35 rokov (M = 26 rokov; SD = 5.27 roku). Hlavnými výskumnými metódami boli denníkové karty, v ktorých účastníčky DBT programu vypĺňali každý deň mieru pociťovaných symptómov súvisiacich s HPO, a tiež použitie jednotlivých DBT zručností v daný deň. K výskumu boli využité okrem denníkových kariet aj online sebaposudzovacie dotazníky zachytávajúce u účastníčok mieru hraničných symptómov, impulzivity, emočnej dysregulácie, disociácie, úzkosti, depresie a všímavosti počas štyroch meraní - na začiatku terapie, v jej strede, na konci terapie a pol roka po DBT terapii. Výsledky z analýzy denníkových kariet ukazujú štatisticky významné zníženie nutkania si ublížiť a počtu sebapoškodení oproti prvej polovici DBT programu. Počet použitých DBT zručností sa oproti prvej polovici programu štatisticky významne zvýšil. Podľa analýzy sebaposudzovacích dotazníkov došlo u účastníčok k štatisticky významnému zníženiu hraničných symptómov, emočnej dysregulácie, depresie, a tiež k signifikantnému zvýšeniu všímavosti. Prezentovaná práca má skôr exploratívnu funkciu vzhľadom na nedostatočné psychometrické vlastnosti denníkových kariet. Aj napriek tomu naše výsledky prispeli k rozšíreniu poznania v oblasti výskumu efektu DBT terapie na pacientov s HPO.
