Resistance genes and plasmids in fluoroquinolone resistant E. coli isolates

Abstract:

V této diplomové práci se věnuji souboru 17 kmenů Escherichia coli, které byly izolovány z výtoku čistírny odpadních vod. Izoláty byly rezistentní k fluorochinolonům, beta-laktamům, ale i řadě jiných antibiotik. Chinolony jsou syntetická antibiotika, která jsou často pouţívána v humánní medicíně pro léčbu močových infekcí způsobených gramnegativními bakteriemi. Rezistence k chinolonům vzniká především mutací v enzymech topoizomeráze IV a DNA gyráze, ale byly objeveny i plazmidově kódované geny rezistence. Čistící proces tedy není dostatečný pro odstranění některých bakteriálních kmenů ze splašků a dokonce mu odolávají i některé druhy antibiotik. Rezistentní bakterie i antibiotika se proto s přečištěnou vodou mohou šířit do povrchových i podzemních vod a půdy. U dvou kmenů byl prokázán přenos plazmidů s geny rezistence k fluorochinolonům konjugací do recipientních kmenů. Bylo detekováno 12 kmenů E. coli patřících do pandemického kmene B2-O25b-ST131, jehoţ výskyt je dokumentován v řadě zemí světa. Klony O25b-ST131 produkují řadu faktorů virulence a jsou častými původci infekcí močového traktu. Tyto klony jsou charakteristické přítomností plazmidů s geny rezistence k fluorochinolonům a geny pro produkci ESBL.

Abstract:

In this thesis I study a set of 17 E. coli strains which was isolated from an outflow of a wastewater treatment plant. Isolates were resistant to fluoroquinolones, beta-lactams and many other antibiotics. Quinolones are synthetic antibiotics which are frequently used in human medicine for treatment of urinary tract infections caused by gramnegative bacteria. Resistance to quinolones is primarly caused by a mutation in topoisomerase IV and DNA gyrase enzymes but the plasmid encoded resistance genes were also detected. The treatment process is not sufficent for the elimination of some bacterial strains from sewage and even some antibiotic types are resistant to it. Resistant bacteria and antibiotics can disseminate with treated water to surface water, ground water and soil. Two strains were positive for a transfer of plasmid with fluoroquinolone-resistance genes by conjugation to recipient strains. Twelve E. coli strains were detected that they belong to the pandemic strain B2-O25b-ST131, which incidence is well documented in many countries in the world. O25b-ST131 clones produce a wide range of virulence factors and they are frequent cause of urinary tract infections. In these clones there is a typical presence of fluoroquinolone resistance genes and ESBL encoded genes carried on plasmids.
