Abstract:

Název: Fyzioterapie u vertebrogenních algických syndromů oblasti bederní páteře Úvod: Práce je rozdělena na teoretickou část, zabývající se problematikou vertebrogenních algických syndromů (VAS), a praktickou část, kde je prezentována kazuistika pacienta s touto diagnózou. U pacientů s VAS je zásadní snížit bolestivost a najít vhodnou metodu fyzioterapie, která zlepší a zastabilizuje zdravotní stav pacienta tak, aby byl schopen fungovat v normálním životě. Cíl: Cílem této bakalářské práce je přispět k řešení problematiky VAS bederní páteře z pohledu fyzioterapie. Metodika: Metodika obsahuje kazuistiku pacienta s VAS v oblasti bederní páteře. Kazuistika zahrnuje úvodní vyšetření pacienta před začátkem terapeutické intervence. Obsahuje testy pohyblivosti páteře, svalové testy a specifické testy jako např. Lasegue, funkční vyšetření posturální stabilizace a reaktibility dle Dynamické neuromuskulární stabilizace, kloubní blokády, svalového tonu v oblasti bederní páteře, Schoberova, Stiborova vzdálenost, Forestierova fleche, Čepojevova vzdálenost, Ottova inklinační a reklinační vzdálenost, Thomayerova vzdálenost a testu lateroflexe. Po vyšetření následovala individuálně orientovaná intervence dlouhá pět týdnů s frekvencí dvou terapií týdně. Intervence byla zaměřena na zlepšení stavu a úpravu pohybových stereotypů pomocí metod Senzomotorické stimulace, Spiraldynamik?, Fyzioterapie funkce, dynamické stabilizace na neurologickém podkladě dle dr. Švejcara a Propriofoot?. Součástí intervence byla i edukace pacienta a manuální techniky. Po pěti terapiích bylo provedeno kontrolní měření vybraných ukazatelů s ohledem na nalezené patologie. Na konci intervence byl vyhodnocen efekt terapie komparací výsledků kineziologického rozboru, který sloužil k sestavení dlouhodobého fyzioterapeutického plánu. Výsledky: Pacientovi se snížila bolestivost z 4/10 na 1/10 při fyzické zátěži, v klidu bolest ani diskomfort nevnímá. Měkké tkáně mají lepší napětí a posunlivost, přítomnost triggerpointů vymizela. Navíc došlo ke zlepšení pohyblivosti páteře, lepší svalové souhře a zlepšení kloubních rozsahů v kyčelním kloubu. Posturální stabilizace byla též zlepšena, nicméně k většímu efektu je třeba se této problematice nadále věnovat a zkvalitňovat dané nastavení. Proto je na místě pokračovat v dlouhodobé fyzioterapeutické intervenci zaměřené na přesun fyzioterapeutických principů do denního pracovního i osobního života. Závěr: Sestavená intervence přinesla očekávaný efekt, nicméně není vyloučena návaznost vpodobném režimu, pokud by chtěl pacient eliminovat bolesti i při náročnějších fyzických aktivitách. Celkově práce autorovi přinesla teoretické prohloubení znalostí v oblasti VAS bederní páteře a možnost tyto znalosti aplikovat u pacienta právě s touto diagnózou.

Abstract:

Title: Physiotherapy in Vertebrogenic Algic Syndromes of the Lumbar Spine Introduction: In patients with VAS, it is essential to reduce pain and to find an appropriate method of physiotherapy that will improve and stabilize the patient's condition to function in normal life. The purpose: The purpose of this bachelor thesis is to contribute to the solution of vertebrogenic algic syndromes of the lumbar spine from the perspective of physiotherapy. Methodology: Contains a case report of a patient with vertebrogenic alginic system in the lumbar spine. The case report includes the initial examination of the patient before therapeutic intervention. The examination includes spinal mobility tests, muscle tests, and specific tests such as Lasegue, functional testing of postural stability and reactivity according to the Dynamic Neuromuscular Stabilization, muscle tone in the lumbar spine, Schober's & Stibor's distance, Forestier's fleche, Cepojova's distance, Otto's inclination and reclination distance, Thomayer's distance, and the lateroflexion test. Examination was followed by an individualized intervention of five weeks in length with therapy sessions occurring twice a week. The intervention focused on improving the condition and modifying movement stereotypes using the methods of Sensomotor Stimulation, Spiraldynamics?, Functional Physiotherapy, Neuromuscular Stabilization, based on Dr. Svejcar's approach and Propriofoot?. The intervention included patient education, myofascial techniques and mobilization. After five therapies, follow-up measurements of selected parameters were performed with respect to the pathologies found. At the end of the intervention, the effect of the therapy was evaluated by comparing the results of kinesiological analysis before and after the intervention from which a long-term physiotherapy plan was subsequently drawn up. Results: Soft tissues have better tension and displacement; the presence of trigger points has disappeared. In addition, there is improved spinal mobility, better muscular coordination and improved joint ranges in the hip joint. Postural stabilization was also improved, however, to have a greater effect this issue needs to be further addressed and continuously improve the patient's condition. Therefore, it is appropriate to continue long- term physiotherapy aimed at transferring activation and exercise principles into daily work and personal life. Conclusion: Overall, the thesis provided the author with a theoretical deepening in the most common musculoskeletal diagnosis, as well as the opportunity to apply the acquired practical and theoretical skills to a patient with this particular diagnosis.
