Abstract:

Transkraniální magnetická stimulace (TMS) je elektrofyziologická metoda, kterou je možno indukovat změny metabolismu v mozkové kůře. V současnosti je TMS využívána jak experimentálně tak rutinně v běžné klinické praxi k funkčnímu vyšetření nervové soustavy. Do centra zájmu se soustřeďuje potenciální terapeutické využití repetitivní TMS (rTMS). Vlastní originální práce byla zaměřena na dvě oblasti zabývající se možností ovlivnění rTMS kognitivních funkcí, motorických symptomů a kortikální excitability u pacientů s Parkinsonovou nemocí a kognitivních funkcí u pacientů s kognitivním deficitem. Ačkoliv se v současnosti repetitivní transkraniální magnetická stimulace jeví jako slibná metoda, zůstává nadále metodou experimentální, kterou v současnosti nelze v neurologii doporučit k rutinní terapii u pacientů s Parkinsonovou nemocí, nebo jako nástroj k rehabilitaci kognitivních funkcí u pacientů s vaskulárním kognitivním deficitem.

Abstract:

Transcranial magnetic stimulation (TMS) is a non-invasive technique, by which is possible to induce metabolic changes in a brain. For the time being, TMS is used as the technique for electrophysiological examination of nervous system in clinical practise as well as for an experimental purpose. A therapeutic potential of repetitive TMS (rTMS) in many psychiatric and neurological disorders is discussed, too. This work focuses on two main issues dealing with a possible effect of repetitive transcranial magnetic stimulation on cognitive function, motor features and cortical excitability in patients with Parkinson´s disease (PD), and cognitive aspects in patients with mild cognitive impairment of the vascular type without dementia (MCI-V). Although the rTMS seems to have a beneficial therapeutic effect in some neuropsychiatric disorders, it cannot be recommended as a routine therapeutical tool either in patients with PD or in patients with MCI-V.
