Klinický význam plazminogen aktivátor systému u pacientů s kolorektálním karcinomem

Clinical significance of plasminogen activator system in colorectal carcinoma patients

Abstract:

Urokináza (uPA) hraje zásadní roli v aktivaci plazminogenu na plazmin, který patří mezi serinové proteinázy a současně se podílí na aktivaci matrixmetaloproteináz, latentních elastáz, růstových faktorů a cytokinů, které se účastní degradace elementů extracelulární matrix. Společně se svým receptorem (uPAR), tkáňovým aktivátorem (tPA) a inhibitory urokinázy (PAI 1, PAI 2, PAI 3 a protese nexin) tvoří plazminogen aktivátor systém (PAS), který je součástí metastatické kaskády a významnou měrou se podílí na invazivním růstu a angiogenezi maligních nádorů. Komplex vitronectin-PAI 1 inhibuje migraci hladkých svalových buněk blokádou alfavbeta3 integrinu k vitronectinu. PAI 1 inhibuje uPA-dependentní invazivitu buněčných linií některých karcinomů. Rostoucí exprese PAI 1 v tkáni některý tumorů je však spojena s horší prognózou. Tím je jeho role v celé šíři nádorového růstu zatím neobjasněná. V rámci našeho projektu byla zkoumána plazmatická hladina PAI 1 a exprese uPA, uPAR, PAI 1 a PAI 2 v nádorové tkáni 80 pacientů s kolorektálním karcinomem. Byl sledován vztah exprese PAS v tumoru a plazmatické hladiny PAI 1 k prognóze a odpovědi na léčbu a dále změny hladiny plazmatického PAI 1 před léčbou a po zahájení terapie kolorektálního karcinomu u jednotlivých stádií. Hodnotili jsme i vztah plazmatické hladiny PAI 1 k nádorovým márkrům CEA a CA19-9. Byly též zkoumány 2 typy genového polymorfismu PAI 1 (4G/5G, 1334G/A) a jejich vztah k prognóze pacientů s kolorektálním karcinomem. Analýzou sebraných dat byla statisticky významně prokázána vyšší exprese uPA, uPAR, PAI 1 a PAI 2 v nádorové tkáni u pacientů v pokročilejším stádiu onemocnění. Dále byla zjištěna vyšší hladina PAI 1 u pacientů s pokročilejším nádorem, která po zahájení léčby významně klesla. Plazmatická hladina PAI 1 byla v pozitivní korelaci k onkomárkrům CEA a CA19-9. Nenalezli jsme však žádnou vazbu mezi genovými polymorfismy PAI 1 (4G/5G a 1334 G/A) ve vztahu ke stádiu a přežití. Závěrem lze říci, že by jednotlivé součásti PAS měly plnit funkci prognostického faktoru a společně se zvýšenou plazmatickou hladinou PAI 1 by měly být brány v úvahu v diagnosticko-terapeutickém procesu pacientů s kolorektálním karcinomem. Zvýšená exprese jednotlivých faktorů PAS v nádorové tkáni by mohla být využita při plánování léčby šité na míru (tailored therapy) a současně by se jeho jednotlivé složky mohly stát terčem cílené léčby.

Abstract:

Urokinase (uPA) plays a fundamental role in transforming plasminogen into plasmin. Plasmin is one of the serine proteases and participates in the activation of matrix metalloproteases, latent elastases, growth factors and cytokines that are involved in the degradation of the elements of the extracellular matrix. Urokinase, together with its receptor (uPAR), tissue activator (tPA) and the urokinase inhibitors (PAI 1, PAI 2, PAI 3 and protese nexin), forms the plasminogen activator system (PAS). PAS is a part of the metastatic cascade and significantly participates in the invasive growth and angiogenesis of malignant tumours. The vitronectin-PAI 1 complex inhibits migration of smooth muscles cells via alphavbeta3 integrine blockade to vitronectin. PAI 1 inhibts uPA-dependent invasivity of some carcinomas. However, an increased expression of PAI 1 in some tumours is linked to more serious prognosis. That is why its role within the whole concept of tumour growth has not been clarified yet. In the course of our project we have studied plasma PAI 1 level and uPA, uPAR, PAI 1 and PAI 2 expression in tumourous tissue in 80 colorectal carcinoma patients. We observed a relation between PAS expression in tumourous tissue and plasma PAI 1 level in connection with prognosis, treatment response and changes of plasma PAI 1 level before the treatment and after the initiation of a therapy of colorectal carcinoma in particular stages. We also studied relationship between plasma PAI 1 level and tumour markers CEA and CA19-9. We investigated 2 types of gene polymorphisms of PAI 1 (4G/5G, 1334G/A) and its relation to prognosis of colorectal carcinoma patients. Analysis of the collected data showed higher expression of uPA, uPAR, PAI 1 and PAI 2 in the colorectal carcinoma tissue of patients with advanced disease. Also, there were detected higher plasma levels of PAI 1 in patients with advanced tumour that significantly decreased after the initiation of the treatment. There was a positive correlation between CEA, CA19-9 and plasma PAI 1 level. However, we did not find any relationship between PAI 1 polymorphisms (4G/5G, 1334 G/A) and the respective stage and survival. It is possible to conclude that the individual parts of PAS factors should play a role as a prognostic factor and together with elevated PAI 1 plasma level should be reflected in diagnostic and therapeutic process of colorectal carcinoma patients. Increased expression of the PAS components in tumour tissue could be followed up when planning tailored therapy and at the same time they could become the targets of the targeted therapy.
