The autonomic nervous system in patients with neurological diseases: cerebral palsy

Abstract:

Disertační práce se zaměřuje na problematiku onemocnění dětská mozková obrna (DMO). Dětská mozková obrna je onemocnění, jež vzniká na základě poškození nezralého mozku v těhotenství, při porodu nebo do 1 roku věku. DMO je definovaná poruchou motorického a posturálního vývoje, který se objeví v časném období po porodu (v návaznosti na strukturální a funkční poškození CNS) a není sice zcela stacionární, ale nemá progresivní ráz. I přes velké pokroky v medicíně je v posledních letech je pozorován vzestupný trend výskytu DMO. Příznaky se liší jedinec od jedince a u téhož nemocného se mohou v průběhu času různě měnit. Někteří lidé s DMO jsou navíc postiženi dalšími zdravotními problémy včetně epileptických záchvatů a poruch mentálních funkcí. Přes všeobecně vžité přesvědčení nemusí znamenat diagnóza DMO vždy automaticky těžký handicap. Zatímco dítě s těžkou formou DMO je mnohdy zcela neschopné chůze a potřebuje velmi náročnou, komplexní a celoživotní péči, dítě s lehkou formou postižení může být jen lehce nemotorné a obejde se bez speciální pomoci. Přes veškerý výzkum není toto onemocnění vyléčitelné, ani neexistuje jeho spolehlivá prevence. Současná medicína přesto nabízí nemocným s DMO pomoc alespoň částečným mírněním některých projevů nemoci vedoucím ke zlepšení jejich kvality života. Včasná identifikace dětí ohrožených vývojem dětské mozkové obrny a jejich zařazení do rehabilitační léčby je zásadní pro kvalitu života postižených.

Abstract:

This thesis is aimal to study the patients with cerebral palsy. We investigated the antropometric parameters that we compared with the available values of healthy population. We examined the measurement of body height and weight and measurement of quality of life related to health (HRQOL). We examined the isometric load using handgrip and analysed blood pressure responses to isometric load. We investigated a set consisted of 83 patients with cerebral palsy, from 42 girls and 41 boys. Investigations were in the age range 8 to 27 years. For testing the quality of life we used a questionaire Kidscreen 52 which includes 52 questions divided to 10 areas: physical activities and health, feelings, general mood, about yourself, free time, family and home life, money matters, friends, school and learning, bullying. We found statistically significant lower value in four areas of the Kidscreen questionaire: physical activities and health, general mood, friends, bullying. Answers to questions in part About yourself we have evaluated statistically significantly higher compared to the healthy population. We noted the differences between the sexes. Girls found the quality of life worse than boys. Our investigation also showed a lower body height and weight in patients with DMO in comparision to healthy subjects. Blood pressure at rest showed higher values of systolic blood pressure and lower diastolic blood pressure than the reference population. For testing the responses of autonomous system we used isometric stress test – handgrip. We found a lower value of maximum handgrip force in patients with DMO in comparison to the healthy population, then we found a higher systolic pressure measured under resting conditions. During isometric stress, we found lower values of blood pressure.
