Use of 4D-CT in Radiotherapy Treatment Planning

Abstract:

Bakalářská práce je zaměřena na využití tzv. 4D-CT pří plánování radioterapie. V teoretické části práce jsou popsány principy dnes již obsolentního dvourozměrného (2D) plánování radioterapie a trojrozměrného (3D) plánování s využitím počítačové tomografie (CT), které se stalo v současné době standardem při plánování zevní radioterapie. Novinkou v plánování radioterapie je využití 4D-CT, které kromě klasického prostorového zobrazení na počítačové tomografii využívá informaci o pohybu jednotlivých orgánů a objemů v čase v souvislosti s dýchacích cyklem. Informace o pohybu orgánů a objemů v rámci dýchacího cyklu jsou velice užitečné pro plánování konformní zevní radioterapie i radioterapie s modulovanou intenzitou pro lepší definici tzv. interního cílového objemu. Navíc, CT data získaná v konkrétní fázi dechového cyklu, např. ve výdechu, je možné využít pro plánování radioterapie metodou tzv. respiratory gatingu, tedy techniky ozařování, které probíhá pouze ve stejné fázi dechového cyklu. V praktické části je ukázán princip a využití 4D-CT na konkrétních pacientech s použitím obrázků z plánovacího systému.

Abstract:

The bachelor thesis is focused on the use of so-called 4D-CT in radiotherapy treatment planning. In the theoretical part, the principle of already obsolete two-dimensional (2D) radiotherapy planning and three-dimensional (3D) radiotherapy planning using computer tomography (CT), which became the standard for external beam radiation therapy planning, are described. The novelty in radiotherapy planning is the use of 4D-CT, which in addition to the classical spatial display on computer tomography uses the information about the movement of individual organs and volumes over time in relation to respiratory cycle. Information about the movement of organs and volumes within the respiratory cycle is very useful for planning of conformal external beam radiotherapy, as well as, intensity-modulated radiotherapy, for a better definition of the so-called internal target volume. In addition, the CT data acquired in a particular phase of the breath cycle, e.g. in exhalation, can be used for planning of radiotherapy method called respiratory gating which takes place only in the same phase of respiratory cycle. In the practical part, the principle and the use of 4D-CT for individual patients using images from the treatment planning system are shown.
