Physiotherapy in arthrosis of joints of the lower limbs

Abstract:

Úvod: Bakalářská práce je zaměřena na oblast fyzioterapie při artróze kloubů dolních končetin. Tato problematika je zvolena z důvodu častého výskytu tohoto problému nejen u starších pacientů, ale i výrazně mladších. Jedná se tedy o aktuální téma. Cíl práce: Cílem mé bakalářské práce je přispět k řešení problematiky fyzioterapie u pacientů s diagnostikovanou osteoartrózou dolních končetin a volbou vhodných fyzioterapeutických postupů, metod a konceptů. Metodika: V této části jsem mezi metody zařadila rozhovor k odebrání anamnézy. Dále provedla vstupní kineziologický rozbor, který zahrnoval pozorování, palpační vyšetření, měření (antropometrie, goniometrie) a testování (svalová síla, zkrácené struktury). Po zpracování vstupního vyšetření jsem navrhla krátkodobý plán fyzioterapie. Pacient s diagnostikovanou artrózou III. stupně v pravém kyčelním kloubu absolvoval 7 terapií, během kterých byly provedeny 2 kontrolní kineziologické rozbory a po 3 měsících vytvořen výstupní kineziologický rozbor s dlouhodobým fyzioterapeutickým plánem. Při práci s pacientem jsem z postupů a metod zvolila techniky měkkých tkání, trakce, míčkovou facilitaci dle Jebavé, strečink, izometrické cvičení, senzomotorickou stimulaci a mezi pomůcky jsem zařadila overball, theraband, Airex podložku, čočku a bosu. Výsledky: Výsledkem terapie při výstupním kineziologickým rozboru bylo zvětšení rozsahu pohybu pravého kyčelního kloubu do aktivní flexe o 25°, kdy při vstupním vyšetření bylo naměřeno 90° a na výstupním vyšetření 115°. Do extenze, abdukce a zevní rotace se zvětšil aktivně i pasivně o 10° a do vnitřní rotace se aktivně zvýšila hybnost o 10°, pasivně o 15°. Došlo k posílení flexorů a extenzorů pravého kyčelního kloubu ze stupně 4 na stupeň 5. Také byly posíleny abduktory ze stupně 3 na stupeň 4. Při antropometrii se prokázala symetrie při měření nad patellou. Podařilo se protáhnout zkrácené struktury a uvolnit hypertonus v daných svalech. Měkké tkáně v oblasti celého pravého stehna a inguíny byly posunlivé a protažlivé. Pohybové stereotypy do flexe a abdukce kyčelního kloubu byly upraveny. Obnovila se i kloubí vůle Závěr: Po společné spolupráci s pacientem byly splněny dané cíle fyzioterapie. Po 3 měsících terapie se především se zvýšil rozsah pohybu, svalová síla, došlo k uvolnění měkkých tkání a hypertonů. Zlepšila se i celková kondice pacienta.

Abstract:

Introduction: The bachelor thesis is focused on physiotherapy in arthrosis of joints of lower limbs. This issue is chosen because of the frequent occurrence of this problem not only in older patients but also significantly younger. It is therefore a current topic. Aim of the work: The aim of my bachelor thesis is to contribute to the solution of physiotherapy in patients with diagnosed osteoarthritis of the lower limbs and choose correct physiotherapeutic procedures, methods and concepts. Methodology: In this section I included an interview to ask for anamnesis. I did an input kinesiological analysis, which included observation, palpation, measurement (anthropometry, goniometry) and testing (muscle strength, shortened structures). After processing the kinesiological analysis I proposed a short-term physiotherapy plan. Patient was diagnosed arthrosis III. He pass out 7 therapies. During these therapies I did 2 kinesiological analyzes. After 3 months I did an output kinesiological analysis and I created a long-term physiotherapeutic plan. While working with the patient I chose soft tissue techniques, traction, ball facilitation according to Jebava, stretching, isometric exercises, sensorimotor stimulation, overball, theraband, Airex pad, and bosu and more. Results: Therapy at the outcome kinesiological analysis resulted increase in the hip joint movement range to active flexion by 25 °. To extension, abduction and external rotation increased actively and passively by 10 ° and into internal rotation movement increased by 10 °, passively by 15 °. The right hip joint flexors and extensors were strengthened from grade 4 to grade 5. Abductors from grade 3 to grade 4 were also strengthened. Anthropometry showed symmetry in measurements above the the knee. We managed to stretch the shortened structures and release the hypertonus in the muscles. The soft tissues around the entire right thigh and inguines were displaceable and elongated. Stereotypes to flexion and abduction of the hip joint were corrected. The joint play of the hip was also recovered Conclusion: After collaboration with the patient our common goals has been achieved. During 3 months the range of movement, muscle strength, soft tissue and hypertones were increased. The patient's condition was also improved.
