Outpatient Childbirth in a Medical Facility

Abstract:

Cíl: Hlavním cílem práce bylo zjistit a popsat názory žen, které ambulantně rodily. Zjistit, jaké typy žen dávají přednost této možnosti porodu. Zmapovat výskyt poporodních komplikací v domácím prostředí a také celkovou spokojenost žen. Metodika: Pro přesné vystihnutí problematiky ambulantního porodu byl zvolen kvalitativní výzkum. Účastníci výzkumu byli vybráni záměrně. Výzkumný soubor tvořilo osm žen po ambulantním porodu. Výzkum vycházel z kvalitativní analýzy získaných dat, která nám pomohla porozumět výpovědím respondentek. Výsledky: Na základě získaných dat byly vymezeny čtyři základní kategorie: 1. Předporodní období, 2. Porod, 3. Poporodní období, 4. Celkové hodnocení. Výzkum ukázal, že o ambulantní porod mají zájem především ženy vzdělané s dobrým sociálním zázemím. Analýza rozhovorů také odhalila, že většina žen si neuvědomuje míru rizika vzniku poporodních komplikací, které se mohou vyskytnout v domácím prostředí. Všechny ženy měly v šestinedělí zajištěnou poporodní péči pro novorozence pediatrem, ovšem pouze ve třech případech byla zajištěna i návštěvní služba porodní asistentky v šestinedělí. Při domácí péči se však nevyskytly žádné závažné zdravotní komplikace. Výsledky výzkumu odhalily, že všechny ženy byly s ambulantním porodem spokojené a v případě dalšího fyziologického těhotenství a porodu by si opět vybraly tuto možnost. Závěr: Česká republika nemá dostatečně propracovaný systém poporodní péče o matku a dítě ve srovnání s ostatními vyspělými státy v Evropské unii. Tento systém nezajišťuje komplexní péči o ženu a novorozence po porodu. Péče soukromé porodní asistentky v šestinedělí je hrazena ze zdravotního pojištění pouze v případě indikace lékaře.

Abstract:

Aim: The aim of this thesis was to evaluate and to describe the opinions of women who gave birth in an ambulant way, to find out those groups of women who prefer this kind of childbirth, to describe an occurence of post-partum complications in home environment and also the overall satisfaction of women. Methods: A qualitative research was made in order to provide an exact report about the matters of the outpatient childbirth. The participants of the research were chosen intentionally. The exploratory group was composed of eight women after the outpatient childbirth. The research was based on qualitative analysis of gained data which helped us to understand the respondents´ answers. Results: Four basic categories were constituted on the basis of the gained data:1. Antenatal period, 2. Childbirth, 3. Post-partum period, 4. Overall evaluation. The research has revealed that the outpatient childbirth is asked for especially by well-educated women living in good social conditions. Furthermore, the analysis of the interviews has shown that most women are not aware of the rate of risk of post-partum complications that can occur in home environment. A pediatrician provided all the women with neonates post-partum care during the period of lying-in. Only in three cases the service of a visiting midwife was provided in the period of lying-in. No serious medical complications ocuured in home environment. Results of the research have revealed that all the women were satisfied with the outpatient childbirth and they stated that in case of another physiological pregnancy and childbirth they would choose this possibility again. Conclusion: The Czech Republic has not a well sophisticated system of the post-partum care for mother and child in comparison with other developed countries of The European Union. The system of the Czech Republic does not provide a complex care for mother and child after a childbirth. Care of the private midwife during the period of lying-in is covered only in case of an indication by a physician.
